Abstract: A actividade física envolve o uso de movimentos repetitivos, que são responsáveis por respostas biomecânicas na estrutura óssea e, em alguns casos, a sobrecarga do esqueleto. Jurmain (1999) propôs que devessem ser criados modelos de forma a testar a relação entre as actividades e os seus marcadores esqueléticos. O objectivo deste estudo é utilizar um modelo gerado a partir da literatura clínica contemporânea e testar as suas correlações com alterações esqueléticas. Para os efeitos do presente estudo, a actividade a ser testada é a costura. A amostra foi seleccionada a partir da Colecção de Esqueletos Identificada de Coimbra e consiste em costureiras, sapateiros e alfaiates (n = 21, cinco indivíduos do sexo feminino e 16 do sexo masculino), e uma amostra de controle “age and sex matched” de igual número. A idade à morte dos indivíduos varia dos 19 aos 96 anos de idade (mediana=45; DP=20,9). Os indicadores registados foram alterações de entese (AE) (método de Coimbra, Henderson et al., 2015; Wilczak et al.,2014), alterações degenerativas articulares (ADA) (Buikstra e Ubelaker, 1994), marcadores de stresse ocupacional (MSO) (presença e ausência) e índice de robustez (RI) (Olivier e Demoulin, 1990). O modelo, criado a partir da literatura citada, indica a presença de alterações ósseas nos membros superiores e inferiores derivadas de movimentos repetitivos circulares e de pedalar utilizados na costura no lado dominante; bem como alterações na coluna vertebral, esterno e coxae, devido à postura durante o trabalho (Arlidge, 1892; Charles, 1893-1894; Lane 1887 e 1888; Milella et al., 2012; Ramazzini, 1703; Thackrah, 1832). Quanto aos erros intra e inter-observadores, estes foram calculados através de percentagens de acordo. Para os dois erros as percentagens de acordo rondam os 70%, havendo algumas mais altas (MSO) e outras mais baixas (IR), ou seja, as percentagens de erro rondam os 30%, um valor razoável de acordo com Henderson et al. (2013a). Análises descritivas e inferenciais foram utilizadas para analisar a assimetria bilateral e a associação entre MOS, AE, ADA e IR com a ocupação, a idade à morte e sexo dos indivíduos e IR. Estas, demonstraram resultados que tanto apoiam como conflituem com outros estudos semelhantes. Em particular, foi encontrada pouca associação entre a ocupação e as variáveis, enquanto que foi encontrada uma forte associação entre as variáveis e idade à morte e sexo dos indivíduos, algo encontrado noutros estudos. No entanto raramente foi encontrada assimetria bilateral, o que contrasta com os achados desses estudos. Da mesma forma, houve uma associação quase inexistente entre as variáveis e o IR, o que contradiz Alves Cardoso (2008). No entanto, boxplots de dispersão entre as variáveis e idade à morte e IR também foram desenhadas, mostrando a presença de “outliers” e áreas de dispersão sobrepostas, o que pode ter causado falsos positivos e negativos nos testes estatísticos inferenciais. Os resultados da assimetria bilateral poderiam ser explicados pela possibilidade de os indivíduos utilizarem tanto o lado esquerdo como o direito na realização das tarefas. Por conseguinte, conclui-se que não existe uma clara associação entre a ocupação e as variáveis analisadas, enquanto foi encontrada uma forte associação para a idade, o que sugere que a própria ocupação não é um factor determinante enquanto a idade e, possivelmente, o stresse físico e mecânico são. Mais importante ainda, encontrou-se presença de MSO específicos para costureiros na amostra de controlo, que eram predominantemente trabalhadores, sendo que os únicos MSO que revelaram uma associação com a ocupação foram os Sawtooth, que estão presentes em costureiros. Assim sendo, parece que o modelo criado a partir da literatura contemporânea britânica não é eficaz ou viável, uma vez que foram encontradas poucas diferenças entre a amostra de costureiros e a amostra de controlo. Novos estudos com modelos criados de forma semelhante e aplicados a uma amostra maior são necessários para melhor compreender o factor da ocupação no desenvolvimento das AE, ADA e MSO.
Activities involve the use of repetitive movements, movements which are responsible for biomechanical responses in bone structure and, in some cases overloading of the skeleton. Jurmain (1999) proposed that models should be created to test the relationship between activities and their skeletal markers. The aim of this study is to use a model generated from contemporaneous clinical literature and to test its associations with skeletal changes. For the purposes of this study, the activity to be tested is sewing. The sample was selected from the Coimbra identified skeletal collection and consists of seamstresses, shoemakers and tailors (n=21, 5 females and 16 males), and an age and sex matched control sample of equal number. The age range of the individuals is 19 to 96 years old (median=45; SD=20.9). The indicators recorded were entheseal changes (EC) (Coimbra method, Henderson et al., 2015; Wilczak et al., 2014), degenerative joint changes (DJC) (Buikstra and Ubelaker, 1994), markers of occupational stress (MOS) (presence and absence) and robusticity index (RI) (Olivier and Demoulin, 1990). The model, created from the cited literature, indicates the presence of bone changes in the upper and lower limbs derived from repetitive circular and pedaling movements used in sewing on the dominant side; as well as changes in the vertebral column, sternum and coxae, due to posture during work (Arlidge, 1892; Charles, 1893-1894; Devereux, 1999; Lane 1887 e 1888; Milella et al., 2012 Ramazzini, 1703; Thackrah, 1832). As for the intra and inter-observer errors, these were calculated by percentages agreement. For both these errors the agreement percentages are around 70%, with some higher (e.g. MOS) and other lower (e.g. IR), which make the percentages of the errors around 30%, an acceptable value according to Henderson et al. (2013a). Descriptive and inferential statistics were used to analyze bilateral asymmetry and the association of MOS, EC, ADA and IR with the occupation, age at death and sex of individuals and IR. These demonstrated results both supported and conflicted with other similar studies. In particular, very little association was found between occupation and the variables, whereas a stronger association was found between the variables and age at death and sex of individuals, which contrasts with the findings of those studies. However bilateral asymmetry was rarely found, which contrasts with other studies. Similarly, there was an almost non-existent association between the variables and the IR, which contradicts Alves Cardoso (2008). However, boxplots of dispersion between the variables and age at death and IR were also plotted, showing the presence of "outliers" and areas of overlapping spread, which might have caused false positives and negatives for he inferential statistical tests. The results of bilateral asymmetry could be explained by the possibility for individuals to use both the left and the right side in the tasks performed, as argued by Blackburn e Knüsel (2006) Knüsel (2000) Rhodes e Knüsel (2005). Therefore it was concluded that there is no clear association between occupation and the variables analyzed, whereas age was found to have a strong association, which suggests that occupation itself is not a determining factor whereas age and possibly the physical and mechanical stress are. Most importantly, it demonstrated the presence of MOS specific to sewers in the control sample, who were predominantly labourers. Thus it appears that the model created from contemporaneous British literature is not effective or viable, since few differences could be found between the sewers sample and the control sample. Further studies with models crated in a similar way and applied to a larger sample size are needed to better understand the occupation factor in the development of EC, DJC and MOS.
