A empatia na consulta e a capacitação dos consulentes

Abstract

Introdução: Apesar de ainda insuficientemente estudada, a empatia assume-se como um elemento crucial quando se trata da relação médico-doente, existindo evidência da sua associação a melhores resultados clínicos. A Medicina Centrada na Pessoa defende a prestação de cuidados de saúde baseada numa partilha de poder entre o médico e o consulente, com vista a uma maior capacitação deste último. Objetivos: Validar para a língua portuguesa, a Jefferson Scale of Patient Perceptions of Physician Empathy (JSPPPE), verificar a sua fiabilidade e estudar a correlação entre a relação médico-doente empática e a capacitação do consulente. Métodos: Foi efetuada uma tradução do questionário JSPPPE para português, segundo as regras internacionalmente aceites para este género de tarefas. O pré-teste da JSPPPE foi apli-cado a 46 consulentes, para validade de conteúdo, pela linguística e conhecimento de problemas de perceção e verificação de fiabilidade. Num estudo observacional, transversal, foram aplicados dois questionários, o JSPPPE e o questionário MCP. A amostra englobou 167 utentes sendo entrevistados consulentes que saíam da consulta com o médico de família, ou aguardavam por esta, na sala de espera da USF Topázio, durante os meses de Agosto e Setembro de 2014. Registaram-se também os dados de cada utente relativamente à idade, sexo, toma diária de medicamentos, grau de instrução e atividade profissional. A empatia na consulta e a capacitação dos consulentes 8 Resultados: Relativamente ao pré-teste para avaliar a fiabilidade da JSPPPE-VP, todas as 5 questões apresentaram um alfa de cronbach superior a 0,836, o que assegura boa fiabilidade. Já no posterior estudo de campo, quanto às pontuações obtidas com a JSPPPE, o valor médio global obtido foi de 6,2. Foram considerados melhores os resultados pertencentes ao percentil igual ou superior ao P75. Sendo que 67,1% dos inquiridos, se encontraram abaixo desse limiar. Quanto ao MCP-PT, a média da pontuação obtida foi de 2,42, sendo que 63,5% dos inquiridos, se encontraram acima do percentil 75. A correlação de Pearson encontrada (0,610) entre os resultados obtidos com a JSPPPE e os obtidos com o MCP-PT, verifica uma associação linear positiva entre estes. Discussão e conclusão: O presente estudo permitiu cumprir os passos necessários a uma tradução fiável da JSPPPE. Concluiu-se que a exploração da perceção do doente sobre a empatia médica é um elemento chave para se melhorar a medicina centrada na pessoa e, deste modo, a capacitação dos consulentes. Background: Although still insufficiently studied, empathy is assumed as a crucial element when it comes to the doctor-patient relationship and there is evidence of its association with improved clinical outcomes. The Centered Medicine in Patient supports the delivery of healthcare based on a sharing of power between the doctor and the user of consultation leading to a greater empowerment of the latter. Objectives: To validate to the Portuguese language, the Jefferson Scale of Patient Perceptions of Physician Empathy (JSPPPE), verify its reliability and to study the correlation between the empathic doctor-patient relationship and the empowerment of the user of consultation. Methods: The JSPPPE questionnaire was translated to Portuguese, according to the internationally accepted rules for this sort of task. The pre-test of the JSPPPE was applied to 46 users of consultations, to content validity, for linguistics, and knowledge of problems of perception and verification of reliability. In an observational, cross-sectional study, two questionnaires were applied, the JSPPPE and the MCP questionnaire. The sample comprised 167 users: consultants that came from consultation with the family doctor, or were waiting for it, were interviewed in the waiting room of USF Topázio, during the months of August and September 2014. Data of each patient regarding to age, sex, daily dose of medicines, education level and occupation were also collected. Results: Regarding the pre-test to assess the reliability of JSPPPE-VP, all 5 questions present an alpha’s cronbach higher than 0,836, which ensures good reliability. In the subsequent field study, in the scores obtained with the JSPPPE the overall average value obtained was 6,2. Discussion and Conclusion: This study fulfilled the necessary steps to reliable translation of JSPPPE. It was concluded that the exploration of the patient's perception about medical empathy is a key element to improve the medicine centered on the person and, thus, the empowerment of users of consultation. The positive association found (Pearson’s correlation) is of sufficient magnitude to warrant further exploration
