perspetiva do doente tem aumentado nos últimos tempos, integrando diferentes índices de
atividade da doença. Em relação à forma como o doente é questionado (Patient Global
Assessment – PGA), várias formulações da questão e período temporal ao qual se reporta esta
avaliação têm sido publicadas na literatura, podendo influenciar a avaliação de atividade e a
decisão terapêutica.
Objectivo: avaliar se a forma como a questão é formulada influencia a resposta do doente e
qual a sua correlação com as outras variáveis do DAS28 e com outras medidas de avaliação
do impacto da doença.
Material e Métodos: Cento e um doentes, com o diagnóstico de AR de acordo com os
Critérios de Classificação ACR 1987 ou ACR/EULAR 2010, seguidos em Consulta de
Reumatologia do Centro Hospitalar e Universitário de Coimbra, completaram de forma
autónoma um questionário que continha sete versões de PGA-VAS em escalas de 100 mm e
duas versões atribuindo um valor de 0 a 100. Avaliaram também fadiga, dor, deformidade e
mobilidade, utilizando escalas visuais 0-100 mm, e a capacidade funcional (HAQ). Pelo
médico assistente foram avaliados os componentes individuais do DAS28. Através do
coeficiente de correlação de Pearson foi avaliada a correlação entre as diferentes PGA-VAS e
entre estas e outras medidas de avaliação do impacto da doença. p<0,05 foi considerado
estatisticamente significativo em todas as análises.
Resultados: As PGA-VAS correlacionam-se fracamente com as outras variáveis do DAS28,
com exceção das articulações dolorosas, numa contagem de 28, em que correlação é
moderada (rho 0.370 a 0.438, p<0.01). Estas correlacionam-se moderada/fortemente com as
outras escalas visuais analógicas (fadiga, dor, deformidade e mobilidade), mas também com a
capacidade funcional do doente (HAQ). Todas as PGA-VAS intercorrelacionam-se moderada/fortemente (rho 0.452 a 0.786, p<0.01), excetuando-se as duas escalas
desenvolvidas pelos investigadores, com a questão realizada na positiva, que têm correlação
fraca ou inexistente com as outras. As diferenças do valor global do DAS28 calculada com as
diversas PGA-VAS é mínima (0.109±0.455).
Conclusão: As diferentes PGA-VAS correlacionam-se moderadamente entre si e com as
outras escalas de avaliação do impacto da doença. O impacto no DAS284vVS utilizando as
várias PGA-VAS foi mínimo. Para uma maior homogenização de resultados recomenda-se a
uniformização da questão da PGA-VAS e a sua validação. Background: Rheumatoid Arthritis´ activity´s evaluation according with the patient´s perspective as assumed a big relevance, incorporating different rates of the disease’s activity. In relation to how the patient is questioned (Patient Global Assessment – PGA), various formulations of the question and the period of in which the evaluation is reported have been published. Objective: In this study, we intend to evaluate if the way the phrasing affects the answer given by the patient and what its correlation with the other parameters of the DAS28 and other measures to evaluate the disease´s impact. Methods: One hundred and one patients, who were fulfilling the 1987 ACR or 2010 ACR/EULAR Diagnostic Criteria for the Rheumatoid Arthritis followed in Rheumatology Department of the Hospital University of Coimbra, completed independently a questionnaire which had seven versions of the PGA-VAS in a scale of 0-100mm and two versions assigning a value from 0 to 100. They also evaluated fatigue, pain, deformity and mobility, using visual scales 0-100mm, and the functional capacity (HAQ). Their medical assistant made an evaluation, a determination of the activity of their disease (DAS28) and their individual components. All PGA-VAS were evaluated using Pearson's correlation coefficient. This has also been used to correlate the PGA-VAS with other variables of the disease’s impact. In allanalysis, p<0.05 was considered statistically significant. Results: The PGA-VAS correlate weakly with the other variables of the DAS28, with the exception of tender joints, score of 28, with which the correlation is moderate (rho 0.370 a 0.438, p<0.01). These correlate moderately/ strongly with other VAS (fatigue, pain, deformity and mobility), but also with the patient's functional ability (HAQ). All PGA-VAS versions correlate moderately/ strongly with each other (rho 0.452 to 0.786, p<0.01), with the exception of the two scales developed by the researchers, where the issue is carried out in an affirmative way, which correlation was weak or inexistent. The difference between the DAS28 global scoring using all PGA-VAS versions was minimal (0.109±0.455). Conclusion: The different PGA-VAS correlate each other moderately and with other scales in which patients self-evaluates of disease impact. The impact in DAS284vVS was minimal. To increase homogenization in the results, we recommend standardizing the question of PGAVAS and its validation. |
