Title: A morfologia óssea como fator de risco na rotura do ligamento cruzado anterior

Abstract: Objetivo O objetivo deste estudo foi analisar diferentes parâmetros identitários da morfologia ósteoarticular entre indivíduos normais e com rotura do LCA, em radiografias convencionais do joelho em perfil, com o intuito de encontrar eventuais associações que possam definir-se como fatores de risco morfológicas.Metodologia Foram estudados 36 doentes (média 28,6 anos; intervalo, 14-55) – 29 homens e 7 mulheres, com rotura recente do LCA (inferior a 6 meses) e radiografia convencional dos seus joelhos. Foram comparados com 36 indivíduos (média 37,9 anos; intervalo 14-58) – 29 homens e 7 mulheres, que funcionaram como grupo de controlo e que tinham radiografias de perfil do joelho sem qualquer patologia osteoarticular. Os parâmetros anatomo-morfológicos das epífises distal do fémur e proximal da tíbia foram medidos na radiografia através do software Corel Draw (Versão 11 © Corel Corporation, 2002, CA). Relativamente aos parâmetros avaliados ao nível do fémur, medimos o diâmetro da diáfise (A), a altura (C) e o diâmetro antero-posterior do côndilo externo (B), o limite inferior da epifise que é aplanado (xy), assim como a distância deste último às corticais anterior e posterior da diáfise, às quais chamamos yz e xw. Quanto aos parâmetros medidos na tíbia, averiguámos o diâmetro antero-posterior dos pratos tibiais (AB), assim como a pentâda da tíbia. Procedeu-se à análise estatística dos parâmetros em estudo (comparação de médias e regressão logística).Resultados Quando comparámos homens sem rotura com mulheres sem rotura identificámos uma altura e um diâmetro ântero-posterior do côndilo externo do fémur maior nos homens sem rotura. Na comparação de mulheres sem rotura com mulheres com rotura do LCA encontrámos uma maior altura do côndilo externo e maior comprimento de xw no fémur, assim como maior diâmetro ântero-posterior dos pratos tibiais, nas primeiras. Na comparação de mulheres com rotura do LCA com homens com rotura do LCA encontrámos no fémur do género masculino maior diâmetro da diáfise, maior diâmetro ântero-posterior do côndilo externo e maior altura do côndilo externo; quanto à tíbia o diâmetro antero-posterior dos pratos tibiais também é maior no género masculino. Quando comparámos homens sem rotura com homens com rotura do LCA encontrámos no fémur dos primeiros uma maior altura do côndilo externo e maior comprimento de yz e xw; no que refere à tíbia, identificámos um diâmetro antero-posterior dos pratos tibiais superior nos indivíduos normais.Conclusão Através deste estudo encontrámos nos homens do grupo de controle diferenças estatisticamente significativas nos parâmetros C, xw, yz e AB comparando com os homens com rotura do LCA. Nos homens do grupo de controle estes parâmetros têm em média um valor superior. Nas mulheres há diferenças estatisticamente significativas nos parâmetros C, xw e AB quando comparámos as mulheres do grupo de controle com as mulheres com rotura do LCA. Nas mulheres do grupo de controle estes parâmetros tem em média um valor superior. Quanto à comparação entre mulheres com rotura do LCA e homens com rotura do LCA, encontrámos diferenças significativas nos parâmetros A, B, C e AB, sendo estes sempre superiores no género masculino. A regressão logística sugere que os parâmetros C, xw, yz, AB e MP2 podem constituir-se como fatores de risco na rotura do LCA. Nesse sentido deverão ser alvo de novos estudos e eventualmente integrados na análise abrangente do perfil de risco lesional para os atletas.
Goal The goal of this study was to analyze different parameters of osteoarticular morphology between normal individuals and in individuals with anterior cruciate ligament rupture on conventional lateral knee radiographs, in order to identify possible associations that can be defined as morphological risk factors.Methods We studied 36 patients (mean 28.6 years; range, 14-55) - 29 men and 7 women, with recent rupture of the ACL (less than 6 months) and with conventional radiography of the knees. We compared them with other 36 subjects (mean 37.9 years, range 14-58) - 29 men and 7 women, who acted as control group and had lateral knee radiographs without any osteoarticular pathology. The anatomomorphological parameters of the distal epiphysis of the femur and proximal epiphysis of the tibia were measured on radiographs using the Corel Draw software (Version 11 © Corel Corporation, 2002, CA). In relation to the femur’s parameters we measured the diameter of the shaft (A), height (C), and anteroposterior diameter of the external condyle (B), the flattened lower limit of the epiphysis and the distance of the latter to the anterior and posterior cortical diaphysis, which we called yz and xw. As for the parameters measured in the tibia, we ascertained the anteroposterior diameter of the tibial plates (AB), as well as the tibial slope. We carried out the statistical analysis of the parameters under study (comparison of means and logistic regression).Results When comparing men with women without rupture we found an increased height and anteroposterior diameter of the external condyle of the femur in men. In comparison between women without rupture with women with rupture of ACL we found a greater height of the external condyle, and a greater length of the xw in the femur, as well as increased anteroposterior diameter of the tibial plates in the control group. When comparing women with ACL rupture with men with ACL rupture we found a largest diameter of the diaphysis, a largest anteroposterior diameter of the external condyle and a largest height of the external condyle in men’s femurs; the tibia and the anteroposterior diameter of the tibial plates are also higher in the men. When comparing men without rupture with men with rupture of ACL we found a largest height of the external condyle and a largest length of the yz and xw in first group’s femurs; in relation to the tibia we found a largest anteroposterior diameter of the tibial plates in normal individuals.Conclusions Through this study we found statistically significant differences in parameters C, xw, yz and AB betwen control group and men with rupture of ACL. In men of the control group these parameters have on average a higher value. Among women there are statistically significant differences in parameters C, xw and AB when comparing women of the control group with women with ruptured ACL. In women of the control group these parameters have on average a higher value. When comparing women with ruptured ACL and men with ACL rupture, we found significant differences in parameters A, B, C and AB, which are always higher in men. The logistic regression suggests that the parameters C, xw, yz, AB and MP2 can be risk factors for the ACL rupture. This parameters should be targets of further studies and eventually integrated in the analysis of athletes’s injury risk profile.
