Title: Relação entre otite seromucosa amamentação e obesidade pediátrica

Abstract: Introdução: A otite seromucosa é uma patologia do ouvido médio muito prevalente na população pediátrica. Existem diversos factores etiológicos implicados no seu desenvolvimento, tais como a obesidade infantil e o aleitamento artificial. Objectivos: Estudar a relação entre otite seromucosa, amamentação exclusiva e obesidade pediátrica. Metodologia: Estudo prospectivo de uma coorte de 189 crianças com idades compreendidas entre os 6 e os 12 anos, submetidas a intervenção cirúrgica por otite seromucosa entre os 3 e os 7 anos de idade. Procurou-se relacionar os percentis de peso e Índice de Massa Corporal (IMC), a existência de amamentação e duração da amamentação exclusiva com a idade mais precoce ou tardia da cirurgia. Resultados: Os percentis de peso e IMC à data da entrevista são significativamente mais elevados do que à data da cirurgia. A maior parte das crianças foram amamentadas, embora durante um período inferior a 6 meses. Não foi determinada nenhuma associação entre a duração da amamentação exclusiva e a idade da cirurgia. Conclusões: Considerando a relação encontrada entre a otite seromucosa e a obesidade, sugere-se que a obesidade infantil seja um factor de risco para otite seromucosa. A maior parte das crianças foi amamentada, contudo com uma duração inferior ao recomendado. O conhecimento mais detalhado dos factores de risco desta patologia constitui a “pedra angular” para a sua adequada prevenção.
Introduction: Otitis media with effusion is a disease of middle ear very common in pediatric population. There are several etiological factors involved in its development, such as pediatric obesity and bottle-feeding. Objectives: Study the relationship between otitis media with effusion, exclusive breastfeeding and pediatric obesity. Methodology: Prospective study of a cohort of 189 children aged between 6 and 12 years old, who underwent surgery for otitis media with effusion between 3 and 7 years. We searched for correlations between the percentiles of weight and Body Mass Index (BMI), the existence of breastfeeding and duration of exclusive breastfeeding with the age of early or late surgery. Results: Weight and BMI percentiles are significantly higher at the time of interview than at surgery. Most children were breastfed, but for less than 6 months. It was not possible to determine association between duration of exclusive breastfeeding and age of surgery. Conclusions: Considering the correlations found between otitis media with effusion and obesity, it is suggested that pediatric obesity is a risk factor for otitis media with effusion. Most of the children were breastfed, yet with a shorter duration than recommended. More detailed knowledge of risk factors of this disease is the “cornerstone” for adequate prevention.
