Esta complicação assenta numa intensa vasoconstrição renal, secundária a um quadro de intensa vasodilatação arterial esplâncnica e a um débito cardíaco reduzido.
O prognóstico é caracteristicamente sombrio, apesar da natureza funcional da síndrome.
Reconhecem-se dois tipos distintos de síndrome hepato-renal: a síndrome hepato-renal tipo-1 que é uma forma aguda com a menor sobrevida e o tipo-2 que é uma forma mais crónica e de prognóstico menos ominoso.
Os mais recentes avanços na compreensão dos mecanismos fisiopatológicos permitiram introduzir novas opções terapêuticas. Os agentes vasoconstritores, como a terlipressina, a midodrina ou a noradrenalina, em associação com a infusão de albumina, proporcionam uma melhoria na função renal, podendo estabelecer uma “ponte” para o transplante hepático.
O transjugular intrahepatic porto-systemic shunt ao reduzir a pressão portal e a actividade simpática associada à hipertensão portal, conduz a uma melhoria da função renal.
Os sistemas de suporte hepático extracorporal, baseados na destoxificação de substâncias vasoactivas veiculadas pela albumina, têm revelado resultados positivos.
A transplantação hepática, no entanto, permanece ainda como o único tratamento efectivo desta entidade clínica. Hepato-renal syndrome is the development of functional renal failure, in patients with advanced stages of liver disease and severe circulatory dysfunction. This complication is characterized by an intense renal vasoconstriction, secondary to an intense splanchnic vasodilatation and reduced cardiac output. Prognosis is extremely poor, despite the functional nature of this syndrome. There are two known different types of hepato-renal syndrome: the type-1 hepato-renal syndrome, which has a rapid evolution and the least survival and type-2 hepato-renal syndrome, which is a more chronic form, with a less ominous prognosis. The most recent advances in the comprehension of the pathophysiological mechanisms, allowed the introduction of new therapeutic options. Vasoconstrictor agents, such as terlipressin, midodrine or norepinephrine, in association with albumin infusion, lead to an improvement in renal function, establishing a bridge for liver transplant. Transjugular intra-hepatic porto-systemic shunt leads to an improvement in renal function, by reducing portal pressure and the sympathetic activity associated with portal hypertension. Extra-corporal liver support systems, based on detoxification of albumin-bound toxins, have shown positive results. Live transplant, however, still remains the only effective treatment of this clinic entity. |
