Abstract: A investigação tem demonstrado que o diagnóstico de uma CCS, como a diabetes, não influencia apenas a QdV da criança/adolescente, mas também a QdV de ambos os pais e a própria dinâmica familiar. Neste sentido, torna-se, igualmente, importante identificar quais os fatores que influenciam quer a QdV das crianças e adolescentes com diabetes quer a adaptação dos seus pais. O presente estudo tem como objetivo analisar a adaptação individual e familiar no contexto da diabetes pediátrica, avaliando o impacto que esta CCS tem na QdV das crianças e adolescentes e dos seus pais, comparativamente a um grupo de controlo. Ainda neste contexto será importante perceber de que forma é que a perceção de sobrecarga familiar/individual dos pais poderá ser um mediador da relação entre a sua sintomatologia ansiosa e depressiva e a QdVS das crianças e adolescentes com diabetes. Neste modelo será adotada uma perspetiva desenvolvimental, sendo testado o efeito moderador da idade da criança. O grupo clínico da amostra deste estudo foi constituído por 72 crianças e adolescentes com diabetes, com idades compreendidas entre os 8 e os 12 anos, e pelos respetivos 116 pais (63 mães e 53 pais). Os participantes deste grupo foram recrutados em contexto hospitalar, numa associação de apoio a jovens diabéticos e através da disponibilização do questionário via online. O protocolo para a avaliação da QdV das crianças e adolescentes com diabetes foi composto pelo DISABKIDS-37 e pelo KIDSCREEN-10. A adaptação psicossocial e a sobrecarga familiar e individual percecionada pelos pais foram avaliadas através do EUROHIS-QOL-8, da EADH e da EIF. Os resultados não revelaram diferenças estatisticamente significativas entre a QdV das crianças/adolescentes com diabetes e as saudáveis. Quando avaliado apenas o grupo clínico, foram os adolescentes os que apresentaram uma pior QdVS e QdVS emocional comparativamente às crianças. No que diz respeito aos cuidadores, foram as mães das crianças e adolescentes com diabetes que apresentaram uma maior sintomatologia ansiosa e depressiva, comparativamente às mães do grupo de controlo e também aos seus parceiros. Relativamente ao modelo proposto, verificou-se que o efeito indireto da ansiedade e depressão materna na QdVS do adolescente, através da sobrecarga familiar/individual percecionada pelas mães, foi moderado pela idade, assim como o efeito direto da depressão na QdVS da criança/adolescente. Estes dados evidenciam que a diabetes tem particular impacto na QdVS dos adolescentes e na adaptação materna. Sugerem também a existência de uma associação entre a adaptação emocional da mãe e a sua perceção de sobrecarga familiar/individual e a QdVS do adolescente com diabetes. Deste modo, os resultados do presente estudo mostram-se importantes não só para a investigação, como também para a intervenção clínica, ao sublinharem a importância da necessidade de promoção de uma melhor adaptação da criança/adolescente à diabetes e ao seu tratamento e de uma melhor adaptação emocional parental.
Research has shown that the diagnosis of a chronic disease, such as diabetes, does not only influence the Quality of life (QOL) of the child/adolescent, but also the QOL of both parents and of the family dynamics. In this sense, it is equally important to identify which factors influence not only the QOL of the children and adolescents with diabetes, but also the adaptation of the parents. The present study’s goal was to analyse the familiar and individual adaptation in the context of paediatric diabetes, assessing the impact that this chronic disease has in the QOL of children, adolescents and their parents. In this same context, it is important to understand in what way may the parents’ perception of the familiar/individual burden, be a mediator of the relationship between their anxious and depressive symptomatology and the QOLs of the children and adolescents with diabetes. In this model, it will be adopted a developmental perspective, testing the moderating effect of the child’s age. The clinical group of this study’s sample was composed by 72 children and adolescents with diabetes, between the ages of 8 and 12, and by their 116 parents (63 mothers and 53 fathers). The participants of this group were recruited in hospital context, in a support association for children and adolescents with diabetes and through the availability of an online questionnaire. The children and adolescents with diabetes QoL assessment protocol, was composed by DISABKIDS-37 and KIDSCREEN-10. The psychosocial adaptation and familiar and individual burden perceived by the parents were assessed through the EUROHIS-QOL-8, EADH and EIF. The results didn’t reveal statistically significant differences between the QOL of children/adolescents with diabetes and the control group. When assessing just the clinical group, the adolescents were the ones who presented a worst HRQOL and emotional HRQOL when compared to children. In what concerns the caregivers, the mothers of the children and adolescents with diabetes, were the ones who presented a higher anxious and depressive symptomatology when compared to the mothers of the control group and their partners. Concerning the proposed model, it was verified that the indirect effect of maternal anxiety and depression in the adolescents’ HRQOL, through the familiar/individual burden perceived by the mothers, was moderated by age, as well as the direct effect of the depression in the child/adolescent HRQOL. These findings suggest that diabetes has a particular impact on adolescents’ HRQOL and maternal adaptation. They also suggest the presence of an association between the emotional adjustment of mothers and their perception of familiar/individual burden and HRQOL of the adolescents with diabetes. Therefore, the results of this study proved to be important not only for research but also for clinical intervention that should promote a better adaptation of the children/adolescents with diabetes and a better parental emotional adjustment.
