Abstract: O presente estudo teve como objectivo averiguar se o Traumatismo Crânio-Encefálico (TCE) afecta a produção de palavras isoladas, numa tarefa de nomeação de imagens, e se esse eventual efeito interage com a sua categoria semântica, extensão, e frequência de uso; averiguar se a idade no momento da lesão tem um efeito directo na magnitude do défice ocasionado pelo TCE na qualidade da produção de palavras isoladas, e de que forma esse efeito é mediado por preditores cognitivos não linguísticos - a Memória de Trabalho (MT), Velocidade de Processamento (VP) e a Inibição; e, por fim, averiguar se cada um desses preditores cognitivos não linguísticos tem efeito directo na magnitude do défice que o TCE ocasiona na qualidade da produção de palavras isoladas, quando controlado o efeito da idade no momento da lesão, ou se, e de que forma, esse efeito é mediado por um, ou vários dos restantes preditores do elenco estudado. Foram estudados dois grupos, um grupo clinico constituído por 27 participantes vítimas de TCE, e um grupo de controlo constituído por 28 participantes saudáveis que nunca haviam sofrido uma lesão cerebral. Os resultados desta investigação revelaram que o TCE não afecta significativamente a produção de palavras isoladas, ainda que se verifique um défice no desempenho do grupo clinico relativamente ao grupo de controlo. Independentemente desse défice verifica-se um efeito significativo da categoria semântica e um efeito significativo de interacção entre a frequência de uso da palavra e a sua extensão na produção oral. A relação dos défices individuais na nomeação nos participantes vítimas de TCE com a Idade no Momento da Lesão, MT, VP e Inibição, foi estudada, apesar de esse défice não ter sido estatisticamente significativo ao nível grupal. Concluiu-se que a idade no momento da lesão tem um efeito directo na magnitude do défice ocasionado pelo TCE na qualidade da produção de palavras isoladas, mas que, se considerarmos as palavras muito frequentes isoladamente, esse efeito é mediado pela Inibição. Por fim, concluímos que a Inibição tem um efeito directo na magnitude do défice que o TCE ocasiona na qualidade da produção de palavras isoladas, independentemente da frequência das palavras, quando controlado o efeito da idade no momento da lesão, mas que o efeito da MT e da VP nesta capacidade linguística é mediado pela capacidade inibitória. Sendo a capacidade linguística tão importante para a integração do Homem na sociedade, bem como uma das funções mais afectadas nas vítimas de lesão cerebral, destacamos a importância desta investigação, não só porque os estudos que abordam esta temática são bastante escassos, como também pela necessidade de alargar esta análise ao nível morfológico e discursivo com o objectivo de ser possível a construção de programas de reabilitação adequados para esta capacidade.
This study aimed to investigate whether Traumatic Brain Injury (TBI) affects the production of single words in a confrontation naming task, and if this possible effect interacts with it semantic category, length and frequency of use; to determine whether age at time of injury has a direct effect on the magnitude of the deficit caused by the TBI in the quality of production of single words, and in what way is this effect mediated by nonlinguistic cognitive predictors - Working Memory (WM), Processing Speed (PS) and inhibition; and, finally, to ascertain whether each of these non-linguistic cognitive predictors has a direct effect on the magnitude of the deficit that TBI causes in the production quality production of isolated words, after controlling for age at time of injury, or whether this effect is mediated by one or more of the remaining predictors studied in the series. Two groups were examined, one consisting of 27 participants with diagnosed TBI and a control group with 28 healthy subjects who had never suffered a brain injury. The results of this research revealed that the TBI does not significantly affect the production of single words, even if there is a deficit in the clinical performance of the group in comparison to the control group. Nevertheless there is a significant effect of semantic category and a significant effect of interaction between the frequency of word usage and extent on vocal output. The relation between individual naming deficits in TCE victims and Age at Time of Injury, WM, PS an Inhibition was investigated, even though such deficits were not significant at the group level. It was concluded that the age at time of injury has a direct effect on the magnitude of the deficit caused by the TBI in the production quality of isolated words. However, if we focus on the words with the highest frequency of use, this effect is mediated by inhibition. Finally, we conclude that inhibition has a direct effect on the magnitude of the deficit that TBI causes the production quality of isolated words, regardless of their frequency, after controlling for age at time of injury, but the effect of WM and PS in this linguistic aptitude is mediated by an inhibitory capability. Since language skills are as important for Man’s integration in society, as they are one of the functions that are affected in most victims of brain injury, we highlight the significance of this research, not only because the studies addressing this issue are scarce, but also by the need to extend this analysis to the morphologic and discourse level in order to be able to create rehabilitation programs that are suitable for this particular ability.
