Translation, Adaptation, and Validation of the “Wake Forest Physician Trust Scale” into European Portuguese (elderly adults’ version)

Abstract

"Introduction: The relationship established between the doctor and patient becomes more meaningful when it is based on trust. Thus, patients can be more vulnerable to expressing their problems and clarifying doubts. Scales such as the Wake Forest Physician Trust Scale, which measures different variables, can be used to evaluate this relationship continuously. Objectives: To cross-culturally adapt and validate the “Wake Forest Physician Trust Scale” (WFPTS) to European-spoken Portuguese, convergently with the “Trust in the Physician Scale” (TiPS), assessing the different variables that influence the establishment of doctor-patient relationships in primary care health centers. Methods: The WFTS was cross-culturally adapted to Portuguese and subsequently validated. This cross-sectional study enrolled 101 adults aged ≥ 65 years via convenience sampling at health centers across central Portugal from September to November 2023. Data were collected on demographic and clinical characteristics, the length of the doctor-patient relationship, and the physician's sex for each participant. The already Portuguese-validated Physician Trust Scale was administered as a concurrent instrument. Statistical analyses included descriptive statistics and evaluation of the psychometric properties of the Wake Forest Physician Trust scale. Results: The sample included 101 patients (65.3% female, 51.5% aged between 65 and 75 years, 47.5% had only completed primary school, 21.8% had secondary school, and 26.7% had university education). WFTS Internal consistency was 0.651 Cronbach’s alpha, items with a value of Cronbach’s alpha if item deleted between 0.597 and 0.689, and two factors explaining 68.577% of the variance of the responses. The TiPS Internal consistency was 0.641 Cronbach’s alpha, items with a value of Cronbach’s alfa if item deleted between 0.589 and 0.689. Regarding the statistical variance of the Trust Physician Scale, one component explained 62.032% of the variance in total responses. The correlation between the Wake Forest Physician Trust Scale and the Trust Physician Scale was positive, moderate, and significant, with a coefficient of correlation of 0.449, p < 0.001. Discussion: These questionnaires are useful for assessing various variables that influence the doctor-patient relationship, with the WFTS being shorter and evaluating more areas of knowledge.Conclusion: The WFPTS was cross-culturally adapted and validated for the European Portuguese-spoken language older population and exhibited satisfactory psychometric properties.Keywords: Trust; Doctor-patient relationship; Older adults; Perspective; General Practice; Primary health care; Family Medicine"

"Introdução: A relação estabelecida entre o médico e o doente torna-se mais significativa quando é baseada na confiança. Desta forma, o doente pode expressar mais vulneravelmente os seus problemas e esclarecer as dúvidas. Para avaliar continuamente essa relação, podem ser utilizadas escalas como a Escala de Confiança do Médico da Wake Forest, que mede diferentes variáveis. Objetivos: Traduzir, validar e adaptar a escala Wake Forest Physician Trust Scale para português, avaliando as diferentes variáveis que influenciam o estabelecimento da relação entre médico e doente, nos centros de saúde. Métodos: A Wake Forest Physician Trust Scale foi adaptada transculturalmente para o português Europeu e posteriormente validada. Para tal foi conduzido um estudo transversal incluindo 101 adultos com 65 anos ou mais, através de amostragem de conveniência em centros de saúde da região centro de Portugal, de setembro a novembro de 2023. Foram recolhidos dados sobre as características demográficas e clínicas, a duração da relação médico-doente e o sexo do médico para cada participante. A Physician Trust Scale, já validada em português, foi administrada como instrumento concorrente. A análise estatística incluiu estatísticas descritivas e avaliação das propriedades psicométricas Wake Forest Physician Trust Scale. Resultados: A amostra incluiu 101 pacientes, 65.3% do sexo feminino, 51.5% com idades entre os 65 e os 75 anos, 47.5% apenas completaram o ensino primário, 21.8% o ensino secundário e 26.7% frequentaram o ensino universitário. A consistência interna da WFTS foi de 0.651 de alfa de Cronbach, com itens com um valor de alfa de Cronbach se eliminados entre 0.597 e 0.689, com 2 fatores a explicar 68.577% da variância das respostas. A consistência interna da TiPS foi de 0.641 de alfa de Cronbach, com itens com um valor de alfa de Cronbach se eliminados entre 0.589 e 0.689. Em relação à variância estatística da Escala de Confiança no Médico, um componente explicou 62.032% da variância nas respostas totais. A correlação entre a Escala de Confiança no Médico da Wake Forest e a Escala de Confiança no Médico foi positiva, moderada e significativa, com um coeficiente de correlação de 0.449, p < 0.001. Discussão: Estes questionários são úteis para avaliar as diferentes variáveis que influenciam a relação entre médico-doente.
