Modificação da Voz na Reatribuição de Género

Abstract

Introduction: The approach to gender dysphoria (DG) and the process of gender reassignment are complex and require multidisciplinary care. Voice modification is one of the biggest challenges in gender reassignment from male to female (M/F), where estrogen therapy proves to be clearly insufficient.Thus, type IV Thyroplasty appears as a surgical procedure that, by raising the fundamental frequency of the voice (F0), allows a satisfactory feminization of the voice (FV). In this context, the aim is to evaluate and compare F0, the patient's satisfaction, her vocal self-perception and the impact her voice has on her daily life, before and after surgery.Clinical Case: Female patient, transsexual, 47 years old, diagnosed with DG, followed at the Genito-Urinary and Sexual Reconstruction Unit (URGUS), was referred to the Otorhinolaryngology (ORL) appointment, due to high frequency voice complaints. She was admitted electively in February 2023 to undergo chondroplasty of the laryngeal prominence and type IV thyroplasty, which were uneventful and with good postoperative evolution. The patient shows, after surgery, an increase in F0, as well as a decrease in the frequency of events that reflect dissatisfaction or negative impact of the voice in her life.Conclusion: Altogether, it is described a clinical case where, surgery combined with speech therapy (TF), allowed a FV, objectified by the change in the patient's F0, reflected in the patient's quality of life, getting closer to her vocal expectations. This reinforces the important role of Otorhinolaryngology in this context.

Introdução: A abordagem da disforia de género (DG) e o processo de reatribuição de género são complexos e exigem cuidados multidisciplinares. A modificação de voz evidencia-se como um dos grandes desafios na mudança de género de masculino para feminino (M/F), onde a terapia com estrogénio se revela manifestamente insuficiente. Surge assim, a Tiroplastia tipo IV, como procedimento cirúrgico que, ao elevar a frequência fundamental da voz (F0), possibilita uma satisfatória feminização da voz (FV). Neste contexto, pretende-se avaliar e comparar a F0, a satisfação da doente, a sua autoperceção vocal e o impacto que a voz tem no seu quotidiano, pré e pós cirurgia.Caso Clínico: Doente do sexo feminino, transsexual, 47 anos, diagnosticada com DG, seguida na Unidade de Reconstrução Génito-Urinária e Sexual (URGUS), foi encaminhada para a consulta de Otorrinolaringologia (ORL), por queixa de voz com frequência grave. Foi internada eletivamente, em fevereiro de 2023, para realização de condroplastia da proeminência laríngea e tiroplastia tipo IV, as quais decorreram sem intercorrências e com boa evolução no pós-operatório. A doente evidenciou, pós cirurgia, um aumento da F0, bem como diminuição da frequência de eventos que traduzem descontentamento ou impacto negativo da voz, na sua vida.Conclusão: Desta forma, descreve-se um caso clínico onde, a cirurgia aliada à terapia da fala (TF), permitiu uma FV, objetivada pela mudança de F0 da doente, refletindo-se na qualidade de vida da mesma, aproximando-se das suas expectativas vocais. Reforça assim, o papel de relevo da Otorrinolaringologia neste contexto.
