O PAPEL DO ASSISTENTE SOCIAL NAS EQUIPAS DE CUIDADOS PALIATIVOS

Abstract

Introdução: Com o envelhecimento populacional e aumento das doenças crónicas e/ou terminais, surgem os cuidados paliativos como uma resposta a tais necessidades. Para que seja possível responder às necessidades da pessoa em contexto paliativo, é necessária a intervenção de uma equipa multidisciplinar, sendo o(a) Assistente Social um dos profissionais integrantes dessas mesmas equipas atendendo a que, a par dos problemas de saúde, existem os problemas sociais a que é preciso dar uma resposta adequada a fim, de se evitar maior sofrimento e mais comorbidades. O Serviço Social surge nos cuidados de saúde, como um optimizador de serviços e recursos, ao diagnosticar e intervir em problemas sociais, evitando que estes se mascarem de problemas físicos ou clínicos.A intervenção do Serviço Social nas equipas de Cuidados Paliativos é essencial e passa por uma intervenção constante junto do doente e da sua família, durante todo o processo da doença e de luto, através da realização de uma avaliação sociofamiliar, onde se identifica a rede de suporte informal e a relação entre os elementos dessa rede. São igualmente identificadas as necessidades psicossociais e preocupações da família, elaborando-se, deste modo, um diagnóstico social e um plano individual de intervenção, atendendo que, em diversas situações, é necessária a intervenção do(a) Assistente Social na orientação e encaminhamento para os recursos da comunidade. Diligências estas que apenas o(a) Assistente Social se encontra habilitado a fazer.Assim, os objetivos deste estudo assentam na análise, avaliação e demonstração da importância e necessidade do Assistente Social nas equipas de Cuidados Paliativos.Metodologia: Estudo observacional, transversal e retrospetivo com abordagem e análise quantitativa e qualitativa a fim de avaliar a intervenção do Serviço Social nos utentes acompanhados pelos serviços de Consulta Externa e de Internamento de Cuidados Paliativos do Centro Hospitalar do Médio Tejo, E.P.E., entre 30 de setembro e 31 de dezembro de 2022. No que respeita à amostra, esta foi constituída pelo número total de doentes que foram acompanhados em Consulta Externa de CP e Internamento de CP (168 utentes) e o número total de doentes que necessitaram de intervenção do Serviço Social (133 utentes), entre o período temporal mencionado. Relativamente aos instrumentos de colheita de dados, estes foram disponibilizados pelos serviços de Planeamento e Controlo de Gestão do Centro Hospitalar do Médio Tejo, E.P.E. e Coordenação do Serviço Social do Centro Hospitalar do Médio Tejo, E.P.E.Resultados e Discussão: Com base na análise dos dados recolhidos, foi possível constatar que o Serviço Social teve intervenção na grande maioria dos doentes acompanhados nos serviços mencionados no CHMT, o que comprova a importância e necessidade do(a) Assistente Social nas equipas de CP. Conclusões: O Serviço Social é um grupo profissional crucial nas equipas de CP, atendendo ao seu trabalho constante junto do doente, família e/ou cuidador e comunidade. Contribui para colmatar as necessidades psicossociais e promove um aumento do bem-estar e qualidade de vida do doente em estado terminal.

Introduction: With population aging and increase in chronic and/or terminal illnesses, palliative care emerges to respond to such needs. In order to be able to answer to the needs of the person in a palliative context, is necessary the intervention of a multidisciplinary team, being the Social Worker one of the professional members of these same teams, considering that, beyond the health problems, there are social problems that it is necessary to give an adequate response in order to avoid greater suffering and more comorbidities. The Social Work comes up, in health care, as an optimizer of services and resources, diagnosing and intervening in social problems, preventing these from masking themselves as physical or clinical problems.The intervention of Social Work in Palliative Care teams is essential and goes through a constant intervention with the patient and his family, during all the illness and bereavement process, by carrying out a sociofamiliar assessment, where is identified the informal support network and the relationship between these elements. Are equally identified the psychosocial needs and concerns of the family, developing in this way, a social diagnosis and an individual intervention plan, considering that, in several situations, the intervention of a Social Worker is necessary at orientation and forwarding for community resources. Steps that only the social worker is qualified to do.Thus, the objectives of this study are based on the analysis, evaluation and demonstration of the importance and the need of the Social Worker in Palliative Care teams.Methodology: Observational, cross-sectional and retrospective study with a quantitative and qualitative approach and analysis in order to evaluate the intervention of the Social Work in the patients followed by Extern Consult of Palliative Care and Palliative Care Hospitalization in Centro Hospitalar do Médio Tejo, E.P.E., between September 30 to December 31, 2022. In respect of the sample, it was constituted by the total number of patients that were followed by Extern Consult of Palliative Care and Palliative Care Hospitalization (168 patients) and the total number of patients who required Social Work intervention (133 patients), between the mentioned time period. Regarding data harvest instruments were made available by the Planning and Management Control services of Centro Hospitalar do Médio Tejo, E.P.E. and Social Work Coordination of Centro Hospitalar do Médio Tejo, E.P.E. Results and Discussion: Based on the analysis of the data collected, it was possible to verify that the Social Work intervened in the most of patients followed in the mentioned services in CHMT, which proves the importance and necessity of the Social Worker in the teams of palliative care.Conclusions: The Social Work is a professional group in the teams of palliative care, considering that to his constant work with the patient, family and/or caregiver and community. Contributes to bridging the psychosocial needs and promotes an increase in the well-being and quality of life of the terminally ill patient.
