dc.description.abstract | Introdução: A Diabetes Mellitus tipo 2 (DM2) e as suas complicações apresentam umaprevalência crescente com elevados custos socioeconómicos e de morbimortalidade. Estetrabalho procura parâmetros antropométricos, laboratoriais e de terapêutica instituída,facilmente acessíveis nos cuidados de saúde primários (CSP), que sejam preditores dodesenvolvimento de doença cardiovascular (DCV), insuficiência renal (IR), retinopatiadiabética (RD) e início de insulinoterapia a 5 anos em doentes com DM2.Métodos: Foi realizado um estudo caso-controlo multicêntrico através da consulta deficheiros de doentes de unidades de saúde familiar (USF) da Administração Regional deSaúde (ARS) do centro, com diagnóstico de DM2 codificado na plataforma SClínico® até31/12/2011. Foram colhidos dados referentes aos anos 2016 e 2021 e analisados através dosoftware SPSS versão 27, onde se obteve a estatística descritiva e inferencial, recorrendo aoteste de Wilcoxon e regressão logística para identificação de preditores das complicações emestudo.Resultados: Numa amostra de 505 indivíduos, foram identificados possíveis preditoresdos outcomes a 5 anos: idade (odds ratio (OR)=1,052) e início de terapêutica antiagregante(OR=3,427) para DCV; idade (OR=1,08), hemoglobina glicada (HbA1c) (OR=1,618), início deterapêutica com diuréticos tiazídicos (DT) (OR=4,092) e diuréticos da ansa (DA) (OR=7,269)para IR; e glicémia (OR=1,015), HbA1c(OR=1,692) e microalbuminúria (OR=1,003) para iníciode insulinoterapia. Foi encontrado um efeito protetor do colesterol high density lipoprotein(HDL) para DCV (OR=0,945) e RD (OR=0,981).Discussão: A relação entre HbA1c, microalbuminúria, glicémia e início de insulinaindiciam um perfil de descontrolo glicémico que indicia falência de células β a 5 anos. Asrelações estabelecidas entre início da toma de antiagregantes e DCV, e entre início de DT,DA e IR, podem ser influenciadas por limitações do estudo, mas levantam a necessidade deestudo do impacto destes fármacos no desenvolvimento de complicações. Encontrou-se umarelação protetora entre colesterol HDL e desenvolvimento de DCV e RD, evidenciando o seupapel antiaterogénico. As principais limitações foram a possível variabilidade na colheita dedados, ausência ou perda de registos e impossibilidade de determinar exatamente quando seiniciou a terapêutica.Conclusão: A HbA1c; microalbuminúria, glicémia, início de terapêutica antiagregante,DT e DA, são possíveis preditores de complicações da DM2 a 5 anos, sobretudo em doentesmais velhos, devendo motivar maior vigilância e ajustes terapêuticos direcionados àprevenção dos outcomes estudados, com vista a protelar o seu desenvolvimento e melhorara sobrevida. | por |
dc.description.abstract | Introduction: Type 2 Diabetes Mellitus (T2DM) and its complications have an increasingprevalence with high socioeconomic costs and morbimortality. This study aims to identifyanthropometric, laboratory, and treatment parameters, easily accessible in primary healthcare, that can predict the development of cardiovascular disease (CVD), kidney failure (KF),diabetic retinopathy (DR), and the need for insulin therapy in 5 years in patients with T2DM.Methods: A multicenter case-control study based on data from patients with a codeddiagnosis of T2DM in the SClínico® platform until 31/12/2011 was conducted in family healthunits of the Associação Regional de Saúde do centro. Data were collected for the years 2016and 2021, and analyzed using SPSS software, Wilcoxon test, and logistic regression.Results: Based on a sample size of 505 patients, statistically significant predictors ofcomplications over 5 years were identified, namely: age (odds ratio (OR)=1.052) and initiationof antiplatelet therapy (OR=3.427) for CVD; age (OR=1.080), glycated hemoglobin (HbA1c)(OR=1.618), initiation of therapy with thiazide diuretics (TD) (OR=4.092) and loop diuretics(LD) (OR=7.269) for KF; and glucose (OR=1.015), HbA1c (OR=1.692), and microalbuminuria(OR=1.003) for the need for insulin therapy. A protective effect of high density lipoproteincholesterol (HDL) was found for CVD (OR=0.945) and DR (OR=0.981).Discussion: The relationship between HbA1c, microalbuminuria, glucose, and insulinrequirement indicates a profile of glycemic control that may signal β-cell failure in the following5 years. The association between initiation of antiplatelet therapy and CVD, as well as theinitiation of TD, LD, and KF, may be influenced by limitations of the study; however furtherresearch is needed to better understand the impact of these drugs on the development ofcomplications. A protective effect of HDL cholesterol for the development of CVD and DR wasfound, supporting HDL’s role as an anti-atherogenic agent. The main limitations of this studyare subjectivity in data collection, absence or loss of records and inability to determine exactlywhen therapy begins.Conclusion: In patients with T2DM, HbA1c, microalbuminuria, glucose, initiation of antiplatelettherapy, TD and LD are potential predictors of complications that may develop within 5 years,particularly in older individuals. Primary care providers can assess these parameters to maketargeted therapeutic adjustments and increase vigilance, ultimately improving patientoutcomes and delaying these complications. | eng |
