dc.description.abstract | O cardioversor desfibrilhador implantável (CDI) consiste num dispositivo ativoimplantável e cirurgicamente invasivo a longo prazo, pertencente à classe III da classificação dos dispositivos médicos. É um sistema que possui a capacidade de detetar e interpretar um sinal biológico ou físico e normalizar a frequência cardíaca através da aplicação um choque elétrico.Os CDIs convencionais são transvenosos (CDI-TV), isto é, os elétrodos de desfibrilhação são inseridos no sistema venoso e terminam nas cavidades cardíacas, permitindo que estes detenham funções como: pacing, desfibrilhação e cardioversão. No entanto, desvantagens associadas aos sistemas tradicionais levaram ao desenvolvimento do CDI subcutâneo (CDI-S), menos invasivo, mas sem função pacing.Nas características de ambos os tipos de CDI existem tanto vantagens como desvantagens, sendo necessário avaliar o doente e as suas necessidades para a seleção do dispositivo mais adequado, existindo ainda a possibilidade de alterar a posição dos eletrocateteres, evitando alguns riscos e limitações associados aos sistemas transvenoso e subcutâneo (posição subesternal).O dispositivo está projetado para a prevenção da morte súbita cardíaca (MSC) e nãopara o tratamento sintomático, pelo que não é de esperar uma melhoria direta da qualidade de vida ou do estado funcional dos doentes. Assim, estes equipamentos são recomendados na prevenção primária e secundária de várias comorbilidades cardíacas.O procedimento cirúrgico para implante do CDI e os follow-up são suscetíveis aorisco de complicações, nomeadamente, associadas ao processo de implante, intrínsecas aos dispositivos e associadas ao recetor do dispositivo; e que podem ser agravadas pela existência de outras comorbilidades.Para além disso, os CDIs são suscetíveis a interferências devido a campos eletromagnéticos (CEM) fortes, visto que os CEM podem ser incorretamente interpretados como atividade elétrica cardíaca intrínseca; e a determinados fármacos que alteram o normal funcionamento do dispositivo.O Farmacêutico possui então um papel preponderante na intervenção junto do doente, principalmente no que diz respeito a esclarecimento, partilha de informação e aassegurar o doente. | por |
dc.description.abstract | The implantable cardioverter defibrillator (ICD) is a long-term, surgically invasive, implantable active device belonging to class III of the classification of medical devices. It is a system that has the ability to detect and interpret a biological or physical signal and normalize the heart rate through the application of an electric shock.Conventional ICDs are transvenous (TV-ICDs), which means that the defibrillationelectrodes are inserted into the venous system and end up in the heart chambers, allowing them to perform functions such as: pacing, defibrillation and cardioversion. However, disadvantages associated with traditional systems led to the development of the less invasive subcutaneous ICD (S-ICD), but without pacing function.The characteristics of both types of ICD constitute both advantages and disadvantages, and it is necessary to evaluate the patient and his needs in order to select the most appropriate device, but there is still the possibility of changing the position of the leads, avoiding some of the risks and limitations associated with the transvenous and subcutaneous systems (substernal position).The device is designed for the prevention of sudden cardiac death (SCD) and not forthe symptomatic treatment, so a direct improvement in the patients' quality of life or functional status is not expected. Thus, these devices are recommended in the primary and secondary prevention of several cardiac comorbidities.The surgical procedure for implantation of the ICD and the follow-up are susceptibleto the risk of complications, namely, associated with the implant process, intrinsic to the devices and associated with the device receiver; and that can be aggravated by the existence of other comorbidities.In addition, ICDs are susceptible to interference due to strong electromagnetic fields (EMF), as EMFs can be incorrectly interpreted as intrinsic cardiac electrical activity; and to certain drugs that alter the normal functioning of the device.Therefore, the Pharmacist has a leading role in the intervention with the patient,mainly with regard to clarification, information sharing and patient assurance. | eng |
