Abstract

This document is in three parts: Health Pharmacy Internship Report, Pharmacy Internship Report and monograph titled “Pharmacy Internship Internship Changes and Maternal Depression During Pregnancy”.The pharmaceutical profession has a very important role in society, and the branch with the greatest representation is the community pharmacy. Health education and promotion is one of the missions that the pharmacist performs in society, and it is increasingly fulfilled due to the expansion and prestige gain of the profession. Pharmacists, as medicine specialists, play a key role in the well-being and care of the population. The internship I performed in a community pharmacy, at Farmácia Frota Carvalho, took place between January and April 2022. The internship frequency report has a SWOT analysis – Strengths, Weaknesses, Opportunities, Threats, in which I evaluate internal and external factors of the internship.As for the internship in the field of pharmaceutical distribution, it took place between May and June 2022 at PLURAL – Cooperativa Farmacêutica, CRL. In the report of this internship I do a SWOT analysis, where I analyze the internal and external factors of the internship.Next, I present the summary of the monograph entitled “Epigenetic changes triggered by maternal stress and depression during pregnancy”.Pregnancy is a period of great vulnerability, in which stress and anxiety can result in depressive conditions. Depression affects 6-17% of pregnant women worldwide and can result in complications such as premature birth and, later, mental health problems in the offspring.The hypothalamic-pituitary-adrenal axis, which culminates in the production of cortisol, the main glucocorticoid in the human body, involves a sequential set of processes that regulate the stress response. The deregulation of this sequence, associated, for example, with stimuli of stress, anxiety or depression, can result in the excessive production of cortisol. In pregnant women, when this dysregulation occurs, and high levels of cortisol reach the fetus during neurodevelopment, changes in genetic programming can occur. Alterations in epigenetic mechanisms, such as DNA methylation and enzymatic alteration of histone proteins, can alter chromatin structure and, consequently, fetal gene expression.In this work I intend to investigate the relationship between maternal stress and depression during pregnancy and epigenetic changes that may occur in fetal DNA, as well as consequences that may occur in the baby's health.

Este documento está divido em três partes: relatório de estágio em farmácia comunitária, relatório de estágio em distribuição farmacêutica e monografia intitulada de “Alterações epigenéticas desencadeadas pelo stress e depressão materna durante a gravidez”.A profissão farmacêutica tem um papel importantíssimo na sociedade, sendo que o ramo com maior representatividade é o da farmácia comunitária. A educação e promoção de saúde é uma das missões que o farmacêutico desempenha na sociedade, sendo que é cada vez mais cumprida devido à expansão e ganho de prestígio da profissão. Os farmacêuticos, enquanto especialistas do medicamento, desempenham um papel fundamental no bem-estar e cuidado da população. O estágio que realizei em farmácia comunitária, na Farmácia Frota Carvalho, realizou-se entre janeiro e abril de 2022. O relatório relativo à frequência do estágio tem uma análise SWOT – Strengths (Pontos Fortes), Weaknesses (Pontos Fracos), Opportunities (Oportunidades), Threats (Ameaças), em que faço a avaliação de fatores internos e externos do estágio.Quanto ao estágio no ramo da distribuição farmacêutica, o mesmo realizou-se entre maio e junho de 2022 na PLURAL – Cooperativa Farmacêutica, CRL. No relatório deste estágio faço uma análise SWOT, onde analiso os fatores internos e externos do estágio.De seguida apresento o resumo da monografia intitulada de “Alterações epigenéticas desencadeadas pelo stress e depressão materna durante a gravidez”.A gravidez é um período de grande vulnerabilidade, em que o stress e a ansiedade podem resultar em quadros depressivos. A depressão afeta 6 a 17% das mulheres grávidas em todo o mundo e pode resultar em complicações como parto prematuro e, mais tarde, problemas de saúde mental na descendência.O eixo hipotálamo-pituitária-adrenal, que culmina com a produção de cortisol, o principal glucocorticoide no organismo humano, envolve um conjunto sequencial de processos, que regulam a resposta ao stress. A desregulação desta sequência, associada, por exemplo a estímulos de stress, ansiedade ou depressão, pode resultar na produção excessiva de cortisol. Em mulheres grávidas, quando ocorre esta desregulação, e níveis elevados de cortisol atingem o feto durante o neurodesenvolvimento, podem ocorrer alterações na programação genética. Alterações de mecanismos epigenéticos, como a metilação do ácido desoxirribonucleico (DNA) e a alteração enzimática de histonas, podem alterar a estrutura da cromatina e, consequentemente, a expressão genética do feto.Neste trabalho pretendo investigar qual a relação entre o stress e depressão materna durante a gravidez e alterações epigenéticas que possam ocorrer no DNA fetal, bem como consequências que possam ocorrer na saúde do bebé.
