dc.description.abstract | Introdução: A esclerose múltipla primária progressiva (EMPP) é o subtipo mais raro da esclerose múltipla, caracterizada, desde início, por acumulação progressiva de incapacidade neurológica. Recentemente, o anticorpo monoclonal humanizado anti-CD20 ocrelizumab foi aprovado como primeiro tratamento na EMPP.O objectivo deste estudo é descrever as características demográficas e clínicas dos pacientes com EMPP no Centro Hospitalar e Universitário de Coimbra (CHUC), e avaliar a efectividade e segurança do ocrelizumab e dos tratamentos off-label utilizados anteriormente. Material e Métodos: Neste estudo observacional retrospectivo, analisámos dados demográficos, clínicos e relativos à terapêutica dos doentes com EMPP seguidos no serviço de Neurologia do CHUC. Resultados: A coorte incluiu 36 pacientes, com idade média 59,8±8,9 anos, dos quais 22 (61,1%) eram do sexo feminino, com duração de doença de 14,9±7,3 anos e mediana de EDSS de 3,5 (2,0-6,0). A apresentação clínica mais comum foi motora, em 35 doentes (97,2%) e a idade média de início de sintomas 45,1±10,5 anos.Treze doentes (36,1%) foram inicialmente tratados com imunossupressores clássicos, 15 (41,7%) com terapêuticas modificadoras de doença (TMD) usadas na esclerose múltipla surto remissão (EMSR), e 6 (16,7%) com ocrelizumab. Durante o tempo de seguimento de 11,4±7,7 anos a taxa global de persistência dos tratamentos foi de 26,5%, sendo que 23,5% dos doentes descontinuaram durante o primeiro ano. A taxa de persistência foi superior no grupo de doentes tratados inicialmente com ocrelizumab quando comparada com as outras TMD (X2=8,127, p=0,017).Discussão e conclusão: As estratégias terapêuticas utilizadas na EMPP foram durante muitos anos off-label e heterogéneas, apresentando baixas taxas de persistência na coorte de doentes deste estudo. Por outro lado, o ocrelizumab apresentou resultados positivos nesta coorte, sobreponíveis aos registados em outros estudos da vida real, parecendo ser eficaz e seguro. | por |
dc.description.abstract | Introduction: Primary Progressive Multiple Sclerosis (PPMS) is the rarest subtype of Multiple Sclerosis, characterized by the progressive accumulation of neurological incapacity from disease onset. Recently, the humanized anti-CD20 monoclonal antibody ocrelizumab was approved as the first PPMS treatment.The aim of this study was to describe demographic and clinical characteristics of a cohort of PPMS patients, and to evaluate treatment effectivity and safety, among previous off-label treatments and ocrelizumab.Material and Methods: In this retrospective observational study, we analysed demographic, clinical and treatment data of patients diagnosed with PPMS at a tertiary center. Results: The cohort included 36 patients, 22 of which were female (61,1%), with mean age of 59,8±8,9 years, length of disease of 14,9±7,3 years, and the baseline Expanded Disability Status Scale (EDSS) was 3,5, ranging from 2,0 to 6,0. The most common clinical presentation was motor, in 35 patients (97,2%), with mean age at symptom onset of 45,1±10,5 years. Thirteen patients (36,1%) were initially treated with immunosuppressive treatment, 15 (41,7%) with disease modifying treatment (DMT) used in Relapsing Remitting Multiple Sclerosis (RRMS), 6 (16,7%) with ocrelizumab. In a mean follow-up time of 11,4±7,7 years, treatment persistence was 26,5%. In the first year, 23,5% of patients discontinued the treatment. Treatment persistence was higher on patients treated with ocrelizumab than with other DMT (X2=8,127, p=0,017).Discussion and conclusion: For many years, the previous treatments used in PPMS were off-label and heterogeneous, showing low persistence in this cohort. In the other hand, ocrelizumab showed positive results in this cohort, as it was described in other real-life studies, and therefore appears to be effective and safe. | eng |
