Abstract: Introdução: A complexidade dos regimes terapêuticos característicos da população idosa, associada ao seu isolamento e a possíveis alterações de natureza física e cognitiva que resultam do processo natural de envelhecimento tornam esta população particularmente vulnerável à não adesão à terapêutica. Por constituir um problema de saúde significativo, é importante conhecer a prevalência desta problemática na população. Objetivo: Este estudo pretende avaliar numa amostra de idosos que vivem isolados na cidade de Coimbra, os seus conhecimentos sobre os medicamentos que tomam e a capacidade de gerir a sua própria medicação nas condições reais em que vivem. Pretende-se que os resultados obtidos venham a servir para a elaboração de um Guia de Boas Práticas na Visita Domiciliária a Idosos Polimedicados. É objetivo específico desta monografia avaliar o funcionamento cognitivo e explorar a adesão à terapêutica na referida amostra. Metedologia: Este estudo foi aplicado no domicílio de 25 idosos de idade igual ou superior a 65 anos e que integram um projeto de apoio social da Câmara Municipal de Coimbra. Para avaliar o funcionamento cognitivo foram utilizadas três questões do Mini Mental State Examination e para avaliar a adesão à terapêutica foi utilizada a versão Portuguesa do Teste de Morisky-Green. Resultados: Na aplicação de três questões do Mini Mental State Examination, 15 (60%) inquiridos responderam corretamente a todas; 4 (16%) e 5 (20%) inquiridos responderam acertadamente a duas e a uma questão, respetivamente; 1(4%) inquirido respondeu de forma errada às três questões. No Teste de Morisky - Green 15 indivíduos (60%) apresentaram pelo menos um comportamento de não adesão, mas nenhum inquirido apresentou um baixo grau de adesão com quatro respostas afirmativas. O comportamento de não adesão mais comum foi descuido e não toma da medicação (n=11; 44%), seguindo-se a dificuldade em se lembrar de tomar a medicação (n=8; 32%). Conclusão: Os resultados deste estudo piloto sugerem que existe um potencial de melhoria da adesão à terapêutica na população inquirida sendo necessário definir intervenções que permitam ultrapassar as barreiras identificadas. O estudo evidência a necessidade de aproximar os profissionais de saúde da população isolada no sentido de melhorar a adesão à terapêutica.
Introduction: The complexity of the medication scheme in the elderly population, in conjunction with isolation and possible physical and cognitive alterations make this population particularly vulnerable to the medication non-adherence. Since it constitutes a significant health problem, it is important to learn the prevalence of this problem in the population. Main Goal: In a sample of elderly people that live isolated in the city of Coimbra, we evaluate their knowledge on the medication they take and their ability to manage their own medication properly in the real conditions in which they live in. The goal of this work is to obtain results that serve to elaborate a Guide of Good Practices for Home Visits to Polypharmacy Elderly People. This study assesses the cognitive functioning and explores the medication adherence in such sample. Methodology: This study was applied in the household of 25 people aged 65 or older and that integrated a social support project from the Câmara Municipal de Coimbra. In order to assess the cognitive functioning three questions of the Mini Mental State Examination were used. To evaluate the medication adherence, the Portuguese version of the Morisky-Green Questionnaire was used. Results: In the response to the three questions from the , 15 (60%) of the Mini Mental State Examination inquired sample answered all questions correctly; 4 (16%) answered correctly to two and 5 (20%) answered correctly to only one question. 1 (4%) answered erroneously to all the three questions. For the Morisky-Green Questionnaire, we observed that 15 (60%) of the inquired sample presented at least one behavior of non-adherence, but none of the subjects questioned presented a low degree of adherence with four affirmative answers. The most common behavior of non-adherence was negligence, and the not taking of medication (n=11; 44%), followed by the difficulty of remembering to take the medication (n=8; 32%). Conclusion: The results of this study suggest that there is a potential to improve medication adherence in the subjects questioned by defining interventions to overcome the identified barriers. This study shows the necessity to bring together the healthcare professionals to isolated populations to improve medication adherence.
