Abstract: Introdução: A hipertensão arterial é um grave problema de saúde pública com uma elevada prevalência em Portugal. Constitui um dos principais fatores de risco no desenvolvimento de doenças cardiovasculares, a principal causa de morte nacional. Torna-se imperioso o adequado seguimento do doente que sofre de hipertensão arterial nos Cuidados de Saúde Primários. O presente trabalho teve como objetivo avaliar a tendência evolutiva do “Índice de acompanhamento adequado de utentes com hipertensão arterial”, na população portuguesa, numa série trienal (2017 a 2019). Para tal, num estudo observacional transversal, os dados do indicador 2013.272.01 FL, constantes na plataforma “Bilhete de Identidade - Cuidados de Saúde Primários”, foram colhidos e as suas dinâmicas de crescimento foram estudadas, desde o total nacional, até ao nível administrativo dos Agrupamentos de Centros de Saúde, para identificar também iniquidades a nível regional. Material e Métodos: Na plataforma online da Matriz de Indicadores dos Cuidados de Saúde Primários, foram selecionados os dados do indicador 2013.272.01 FL. Estudaram-se os resultados existentes à data de 31 de dezembro de 2017, 2018 e 2019, por total nacional, Administração Regional de Saúde e dentro destas, por Agrupamentos de Centros de Saúde, calculando a média dos aleatorizados. Definiu-se para a dinâmica de crescimento um valor limite mínimo de (∆) ≥+1,5%. Realizou-se estatística descritiva e inferencial. Resultados: O índice de acompanhamento adequado de utentes com hipertensão arterial aumentou de 0,657 em 2017, para 0,677 em 2019 (∆=+3,0%) no triénio 2017-2019. Nesse período, na análise por região identificou-se diferença estatisticamente significativa (p<0.01) na tendência evolutiva do índice de acompanhamento dos utentes hipertensos. Na análise por Agrupamentos de Centros de Saúde, apenas para a Região de Saúde do Centro foi encontrada diferença estatisticamente significativa (p<0.05) na dinâmica de crescimento do indicador 2013.272.01 FL. A evolução crescente do índice foi mais evidente na região do Algarve (dinâmica de crescimento de 10,3%), em oposição à região do Norte com uma evolução mais ténue (dinâmica de crescimento de 1,5%). O índice de acompanhamento do utente hipertenso verificou-se ser maior a norte que a sul de Portugal. Discussão: Foram encontradas assimetrias entre regiões na evolução do índice de acompanhamento adequado dos utentes com hipertensão arterial. Verificaram-se dinâmicas de crescimento positivas, excetuando no Alentejo, que apresentou degradação do índice no triénio em estudo. Conclusão: A tendência evolutiva do índice de acompanhamento adequado dos utentes com hipertensão arterial nas regiões do Algarve, Centro e Lisboa e Vale do Tejo superou o crescimento determinado como adequado. Nas restantes ficou aquém do desejável. O conhecimento das assimetrias regionais é um elemento fulcral para o entendimento de falhas que poderão ser colmatas com uma melhor alocação de recursos e implementação de estratégias. A criação de indicadores, inerentes às variáveis do score de acompanhamento, bem como a grupos etários, será importante para melhor compreender os resultados e identificar os procedimentos em défice na consulta de vigilância da Pessoa que sofre de hipertensão arterial.
Introduction: Hypertension is a serious public health problem with a high prevalence in Portugal. It constitutes one of the main risk factors in the development of cardiovascular diseases, the main cause of national death. Adequate follow-up of the patient suffering from arterial hypertension in Primary Health Care is imperative. The present study aimed to evaluate an evolutionary trend of the “adequate follow-up index of patient with arterial hypertension”, in the Portuguese population, in a three-year series (2017 to 2019). For this purpose, in an observational cross-sectional study, data from the 2013.272.01 FL indicator present in the “Portuguese Primary Health Care - Identity Card” platform were collected and its growth dynamics were studied, from the national figures, down to the lowest administrative levels, the Health Centre Clusters, to also evaluate inequities at regional level. Material and Methods: From the Primary Health Care Indicators matrix online platform, 2013.272.01 FL indicator data were selected. The results existing on December 31st for 2017, 2018 and 2019 were studied by National level, Regional Health Administrations level, and within these, by Health Centre Clusters levels, calculating the average of the randomized. A cut-off limit of de (∆) ≥+1,5% was defined. Descriptive and inferential statistics were performed. Results: Adequate follow-up index of patient with arterial hypertension increased from 0.657 in 2017, to 0.677 in 2019 (∆=+3.0%) in the 2017-19 three-year. During this period, the analysis by region identified a statistically significant difference (p <0.01) in the evolutionary trend of adequate monitoring index of hypertensive patients. In analysis by Health Centre Clusters, only for Center Health Region, a statistically significant difference (p <0.05) was found in 2013.272.01 FL indicator growth dynamics. The increasing evolution of index was more evident in Algarve region (growth dynamics of 10.3%), in contrast to the North region with a more tenuous evolution (growth dynamics of 1.5%). The follow-up index for hypertensive patient was higher in the north than in the south of Portugal.Discussion: Some asymmetries were found in the analysis by region in evolution of the adequate follow-up index of patient with arterial hypertension. Positive growth dynamics were observed, except in Alentejo, which showed a degradation of index during the study period. Conclusion: Evolutionary trend of the adequate follow-up index of patient with arterial hypertension in the regions of Algarve, Centre and Lisbon and Tagus Valley surpassed growth determined as adequate. In the others regions, it was less than desirable. The knowledge of regional asymmetries is a key element for the understanding of failures that can be overcome with a better allocation of resources and implementation of strategies. Creation subcategories of indicators, inherent to variables of the monitoring score, as well age groups subcategories, will be important to better understand the results and to identify procedures in deficit in surveillance consultation of Person suffering from arterial hypertension.
