Abstract: O presente trabalho engloba dois relatórios de estágio, onde é analisado o estágio curricular que realizei na Autoridade Nacional do Medicamento e Produtos de Saúde, I.P. - INFARMED, I.P., decorrido de janeiro a março de 2020, assim como o estágio em Farmácia Comunitária, na Farmácia Cortesão, entre maio e setembro de 2020. Contém, também, a monografia intitulada "Inibidores PD-1/PD-L1 no tratamento do cancro". O cancro é considerado a segunda principal causa de morte a nível mundial. A compreensão do envolvimento do Sistema Imunitário no processo de crescimento e progressão do tumor, com a descoberta de mecanismos utilizados para escapar à vigilância imunitária, veio revolucionar o tratamento de muitos cancros que se apresentam refratários às terapêuticas convencionais e que, frequentemente, possuem reduzidas alternativas terapêuticas. Uma das possíveis abordagens que permite a restauração da resposta imunitária antitumoral consiste na inibição de immune checkpoints. A Programmed cell death 1 (PD-1) e Programmed death ligand I (PD-L1), recetor e ligando respetivamente, pertencem a esta classe de moléculas, e são responsáveis pela emissão de sinais negativos que culminam na supressãoda resposta imunitária antitumoral. Por conseguinte, surgem os inibidores PD-1 e PD-L1 que, em relativamente pouco tempo, conseguiram conquistar o seu território no tratamento do cancro, com eficácia comprovada que se reflete em inúmeras aprovações, em diferentes indicações terapêuticas, inclusive em primeira linha. Neste trabalho é detalhado o papel da via PD-1/PD-L1 na sobrevivência do tumor e é realizada uma revisão da utilização dos inibidores PD-1 e PD-L1 na prática clínica, nos diferentes tipos de cancro, quer em monoterapia, quer em combinação com outras estratégias terapêuticas. Adicionalmente, são abordadas as questões de segurança associadas à sua utilização bem como o papel da PD-L1 como biomarcador preditivo.
The present work includes two internship reports. The first report analyses the curricular internship that took place at the National Authority of Medicines and Health Products, I.P - INFARMED, I.P., from january to march of 2020, and the second one describes the curricular internship in Community Pharmacy, at Farmácia Cortesão, from may to september of 2020. This work also comprises the monograph entitled "PD-1/PD-L1 inhibitors for cancer treatment". Cancer is the second leading cause of death worldwide. The comprehension of the Immune System involvement in the growing process and progression of the tumor, with the discovery of mechanisms by which tumors evade immune control, changed the cancer treatment paradigm where a variety of cancers are refractory to conventional treatments and frequently have reduced therapeutic options. A possible approach for the antitumoral immune response reactivation is the blockade of immune checkpoints. Programmed cell death 1 (PD-1) and Programmed death ligand I (PD-L1), respectively receptor and ligand, are part of this class of molecules and are responsible for the transmission of negative signals that end up suppressing the antitumoral immune response. Thus, PD-1 and PD-L1 inhibitors emerged and, in a short period of time, gained significant awareness in the cancer treatment field, with a great amount of approvals, including in first line treatment. The present work explains the role of the PD-1/PD-L1 pathway in the tumor survival mechanism and reviews the application of PD-1 and PD-L1 inhibitors, alone or combined with other therapeutic strategies, in clinical practice, regarding different types of cancer. Additionally, safety concerns are presented and the use of PD-L1 as a predictive biomarker is discussed.
