Reimplante Intencional: uma alternativa a considerar

Abstract

Introdução: O reimplante intencional apresenta-se como uma terapêutica alternativa conservadora e económica, quando as modalidades de retratamento endodôntico convencionais são impraticáveis, falharam ou se associam a riscos que o paciente recusa. O objetivo do presente estudo é fazer uma revisão bibliográfica sobre este procedimento, assinalando as suas indicações e riscos, discutindo os protocolos clínicos recentes, bem como as técnicas e materiais subjacentes, identificando fatores com impacto no prognóstico. Métodos: Para a realização desta revisão procedeu-se à pesquisa bibliográfica recorrendo às bases de dados eletrónicas Pubmed, Embase e Cochrane, utilizando a palavra-chave: Reimplante dentário. Tendo sido aplicados os filtros de pesquisa nos últimos 10 anos e em língua inglesa, portuguesa ou espanhola.Resultados: Após a seleção, por critérios de inclusão e exclusão, foram selecionados 42 artigos e 2 livros para a elaboração do presente estudo.Discussão/Conclusão: A técnica de reimplante intencional tem vindo a ser progressivamente modificada e refinada, envolvendo, atualmente, métodos de extração dentária atraumáticos, a ressecção e preparação radicular com sistemas piezoelétricos, a manipulação extra-oral durante o menor período possível, num meio adequado à manutenção da viabilidade celular, e a obturação com recurso a diferentes biomateriais, executadas sob iluminação e ampliação. Estudos recentes relatam taxas de sobrevivência elevadas. Conclui-se, assim, que o reimplante intencional representa uma boa opção de tratamento, cada vez mais aceite e difundida para a abordagem da patologia periapical pós-tratamento.

Background: Intentional replantation is an alternative which provides a more economical and conservative therapy, when conventional endodontic retreatment modalities are unfeasible, failed or are associated with risks that patient refuses. The aim of this study is not only to document the surgical procedure, but also to discuss its indications and risks. In addition, it is also intended to describe the techniques and materials used and explore predictors of the outcome.Methods: A search of the literature was performed using electronic databases Pubmed, Embase and Cochrane, using the keyword: Tooth replantation. The search filters have been applied in the last 10 years and in English, Portuguese or Spanish.Results: After the selection, by inclusion and exclusion criteria, 42 articles and 2 books were included for the elaboration of the present study.Discussion/Conclusion: The intentional replantation technique has been progressively modified and refined, currently involving methods of atraumatic tooth extraction, resection and root preparation with piezoelectric systems, extra-oral manipulation during the shortest possible period, in an appropriate medium to the maintenance of cell viability and filling with different biomaterials, performed under illumination and magnification. Recent studies report high survival rates. Therefore, it’s concluded that intentional replantation represents a good treatment option, which is increasingly accepted and diffused to manage post-treatment endodontic disease.
