Abstract: Introdução: Adiar a maternidade tem sido uma tendência crescente, razão pela qual se tem demonstrado a importância de providenciar informação personalizada relativamente ao estado da fertilidade individual, permitindo às mulheres tomarem decisões informadas e conscientes relativamente às intenções de parentalidade e planeamento da primeira gravidez.Objetivos: O objetivo deste estudo foi avaliar o potencial reprodutivo duma pequena população de mulheres saudáveis e aconselhá-las de acordo com os resultados, avaliando o efeito desta informação nas suas escolhas relativamente à maternidade e reprodução. Avaliou-se também o seu conhecimento relativamente a fertilidade e as suas intenções perante técnicas de preservação de fertilidade.Materiais e métodos: Tratou-se de um estudo prospetivo intervencional. Foi oferecida avaliação e aconselhamento reprodutivo baseado na avaliação da reserva (contagem de folículos antrais e hormona anti-mülleriana) e função ovárica (FSH, estradiol, progesterona e prolactina) no 3.º e 23.º dias do ciclo. Um questionário foi aplicado em dois momentos distintos, para verificar alterações nas intenções reprodutivas e parentais. Doze mulheres, entre os 21 e os 33 anos, aceitaram participar no estudo. Resultados: Inicialmente, as mulheres planeavam ter o primeiro filho por volta dos 32,17 anos (DP ± 2,517), mas depois do estudo esta idade diminuiu cerca de 1 ano (média = 31,33, DP ± 2,605, t(11) = 3,079, p = 0,010). A progressão na carreira foi apontada como o principal obstáculo quanto a ter filhos mais cedo. O conhecimento de fertilidade era alto. 25% das participantes apresentavam alterações da função e reserva ovárica. Metade das participantes enquadrava-se no grupo de risco. É menos provável que as participantes de baixo risco antecipem a idade em que terão o primeiro filho (p = 0,044) ou recorram a criopreservação de ovócitos (p = 0,008) do que as mulheres do grupo de risco. Metade das participantes consideraria criopreservar os seus ovócitos, e duas delas avançaram com esta decisão.Discussão: As participantes ficaram mais tranquilas relativamente à sua fertilidade, o que pode ser contraproducente, considerando que se baseiam em achados que, apesar de fiáveis, não são absolutos. Existem também limitações naquilo que podemos oferecer às mulheres que procuram este tipo de aconselhamento. Devem realizar-se mais estudos em populações mais diversificadas. O nosso estudo mostra que este tipo de avaliação e aconselhamento reprodutivo permite que as mulheres tomem decisões mais informadas relativamente à sua fertilidade, apesar de não estar claro o efeito desta tomada de decisão na concretização dos projetos parentais. Os nossos achados reforçam a necessidade destas iniciativas serem oferecidas precocemente e a um maior número de pessoas.
Introduction: Postponing motherhood has been a rising trend, which highlights the need for providing counselling regarding individual fertility assessment, allowing women to make informed decisions regarding their parenting and childbearing intentions.Objectives: The aim of this study was to evaluate the fertility status of a small population of healthy women, and provide counselling accordingly to the results, finding if this information leads to changes in their life choices regarding reproduction and childbearing. Their knowledge about fertility and their intentions towards fertility preservation were also be evaluated.Materials and Methods: This was a prospective interventional study. We offered individual fertility assessment and counselling based on evaluation of the ovarian reserve (antral follicle count and anti-müllerian hormone) and function (FSH, estradiol, progesterone e prolactine) in the 3rd and 23rd days of the cycle. A survey was applied in two different moments to evaluate the changes in the reproductive and parenting intentions. Twelve women, between 21 and 33 years old, took part in the study.Results: Initially, participants planned to have their first child it at 32,17 (SD ± 2,517), but after entering the study, this age decreased about 1 year (M = 31,33, SD ± 2,605, t(11) = 3,079, p = 0,010). Career progression was the main reason pointed out for delaying childbearing. Their fertility knowledge was high. 25% of participants presented with alterations in the ovarian reserve and function. Half of the participants were ranked as high risk, and low risk participants were less likely to anticipate the age for childbearing than those in the high risk group (p = 0,044), as well as less likely to use egg freezing techniques, (p = 0,008). Half of the participants said they would consider freezing their eggs, as 2 of them went forward with this decision. Discussion: Participants were more reassured about their fertility, which can be counterproductive, as they are relying on findings that, despite reliable, are not absolute. There also limitations in terms of what can be offered to women who seek this kind of counselling. Further studies should be applied in more diverse populations. Our study demonstrates that this type of individual fertility assessment empowers women to make more informed decisions regarding their fertility, despite that the effects of these decisions in the fulfilment of their reproductive plan are not clear. Our findings reinforce the need for these initiatives to be applied earlier and to a more extended population.
