Title: A síndrome de anemia cardiorrenal no idoso : efeitos citoprotectores da eritropoietina

Abstract: Introdução: Nos países desenvolvidos, pela melhoria das condições higieno-sanitárias e dos cuidados de saúde, assiste-se a um gradual envelhecimento da população, com aumento da prevalência de doenças associadas à idade avançada/senilidade. O conceito de síndrome de anemia cardiorrenal é recente e mostra como a anemia, a insuficiência cardíaca e a doença renal crónica se correlacionam, estabelecendo uma tríade em que o agravamento de cada uma das condições contribui para o agravamento das outras, formando um círculo vicioso. Objectivos: Com este trabalho de revisão pretende-se uma melhor compreensão dos efeitos deletérios da anemia no idoso, bem como da sua relação com a insuficiência cardíaca e a doença renal crónica. A partir deste ponto, procura-se perceber como a terapêutica da anemia, recorrendo à eritropoietina recombinante humana, pode ser benéfica na interrupção do círculo vicioso característico da síndrome de anemia cardiorrenal através dos seus conhecidos efeitos como estimulante da eritropoiese, bem como dos seus efeitos não ertitropoiéticos benéficos, considerando as comorbilidades frequentemente encontradas nos idosos. Desenvolvimento: A eritropoietina recombinante humana tem um efeito protector na função cardíaca e renal, com melhoria da fracção de ejecção ventricular esquerda e com atraso da progressão da doença renal, bem como com redução da sintomatologia. Também há evidência da redução dos sintomas provocados pela anemia em si, com consequente melhoria da qualidade de vida dos doentes. Há relatos da ocorrência de alguns efeitos adversos da terapêutica com eritropoietina recombinante humana, nomeadamente hipertensão arterial e eventos tromboembólicos, contudo evidências sugerem que a sua ocorrência está na dependência da dose de fármaco utilizada. Conclusões: Pela observação dos dados disponíveis na literatura compreende-se que a utilização da eritropoietina na síndrome de anemia cardiorrenal é vantajosa, contribuindo não só para a melhoria da situação clínica dos doentes mas também para o atraso da progressão da patologia já instalada. É, contudo, necessária a realização de novos estudos que permitam concluir sobre a relação dos efeitos adversos com a dose utilizada, com a finalidade de optimizar a terapêutica, aumentando o nível de segurança. Para qualquer caso, deve ser contrabalançado o benefício-risco desta terapêutica. Existem, actualmente, novos agentes estimulantes da eritropoiese sob investigação, no sentido de melhorar, não apenas a comodidade da administração e reduzir o custo da terapêutica, mas também potenciar os efeitos citoprotectores.
Introduction: In the developed countries, due to the improvement of the hygiene and sanitary conditions, as well as the health care, there’s a progressive ageing of the populations, with a significant increase of the prevalence of age-associated diseases. The concept of cardiorenal anemia syndrome is recent and shows how anemia, heart failure and chronic kidney disease are interconnected, establishing a triad in which the worsening of one of the conditions contributes to the deterioration of the others, forming a vicious cycle. Objectives: This revision intends to better understand the deleterious effects of anemia in the elderly, as well as it relationship with heart failure and chronic kidney disease. From this point, it seeks to understand how treatment of anemia with recombinant human erythropoietin is beneficial in the suspension of the typical vicious cycle of cardiorenal anemia syndrome through its effects in erythropoiesis, as well as the beneficial non-erythropoietic effects, considering the frequent comorbidities in the elderly. Development: Several studies have shown the protective effect of recombinant human erythropoietin in cardiac and renal function, with improvement of left ventricular ejection fraction, delay on the progression of kidney disease, as well as with reduction of the symptoms. There’s also evidence of the reduction of the symptoms caused by anemia itself, with improvement of quality of life of the patients. Reports show the occurrence of some adverse effects of the treatment with recombinant human erythropoietin, namely arterial hypertension and thromboembolic events. However, other studies suggest that those effects are related to the dose of the drug. Conclusions: Considering the available data in the literature, it can be understood how the use of erythropoietin in the treatment of cardiorenal anemia syndrome is beneficial, contributing not only for the improvement of the clinical situation of patients, but also for the delay of the already established pathology. Nevertheless, more studies are necessary, so the relationship between the adverse effects and the applied dose of medicine is better understood, in order to adjust the therapy, increasing safety. The risk-benefit should always be evaluated in every case. Currently, there are some new erythropoiesis stimulating agents under investigation, in an effort, not only to improve the convenience of the administration and reduce the cost of the treatment, but also to potentiate the beneficial non-erythropoietic effects
