Abstract: O presente documento foi elaborado no âmbito da unidade curricular “Estágio Curricu-lar” do Mestrado Integrado em Ciências Farmacêuticas para ser apresentado à Faculdade de Farmácia da Universidade de Coimbra com vista à obtenção do grau de Mestre.O documento divide-se em três partes contemplando a monografia, intitulada “Iatroge-nia de quinolonas: uma análise de notificações espontâneas” e realizada sob orientação do Professor Doutor Carlos Alves, bem como os relatórios referentes aos estágios realizados em farmácia comunitária, na Farmácia Machado sob orientação do Dr. João Maia, e em far-mácia hospitalar, na Administração Regional de Saúde do Centro sob orientação da Dr.ª Ca-tarina Coelho.A monografia foi realizada com base em revisão bibliográfica de literatura relevante para o tema e na análise de notificações espontâneas de reações adversas aos antibióticos perten-centes à classe das quinolonas reportadas à Unidade de Farmacovigilância de Coimbra, AIBI-LI (Associação para Investigação Biomédica em Luz e Imagem), num horizonte temporal de cinco anos. As quinolonas são uma classe de antibióticos sintéticos de amplo espetro que atuam por inibição da síntese do ADN, amplamente utilizadas no tratamento de infeções bacterianas, o que incitou ao incremento de resistências a este grupo. Para análise da iatro-genia provocada por estes fármacos, foram consideradas 29 notificações espontâneas con-tendo 51 eventos adversos. Os casos foram caracterizados tendo por base critérios de vali-dação primários (descrição da(s) RAM(s), identificação do(s) medicamentos(s) suspeito(s), identificação do doente e do notificador) e secundários (gravidade, causalidade e conheci-mento prévio). O fármaco mais frequentemente suspeito foi a ciprofloxacina (n=19; 65,52%). Do total de notificações, 6,90% continham eventos adversos simultaneamente graves, desco-nhecidos e com relação pelo menos provável com o medicamento suspeito, facto que traduz a importância da NE na geração de novo conhecimento acerca do perfil de segurança dos medicamentos. O desenvolvimento de novas quinolonas aliado ao conhecimento gerado nesta área permitem um alargamento de opções terapêuticas e do perfil de segurança dos medicamentos comercializados.Os relatórios de estágio possuem estrutura de uma análise SWOT (Strengths, Forças; Weaknesses, Fraquezas; Opportunities, Oportunidades e Threats, Ameaças), relativamente à minha opinião pessoal acerca da adequação do plano curricular face às perspetivas profissio-nais futuras, aos conhecimentos adquiridos neste primeiro contacto com a prática profissio-nal, às atividades desenvolvidas no seio dessas instituições e aos seus modi operandi. Desta observação concluiu-se que, apesar do serviço de excelência prestado por estas entidades, existem aspetos que poderão ser melhorados.
This document was written within the scope of the curricular unit “Internship” of the Masters in Pharmaceutical Sciences to be presented to the Faculty of Pharmacy of the Uni-versity of Coimbra to obtaining a Master's degree.The document is divided into three parts, including the monograph entitled "Iatrogenesis of quinolones: an analysis of spontaneous reports" and conducted under the guidance of Profes-sor Dr. Carlos Alves, as well as the reports on the internships in community pharmacy at guidance of Dr. João Maia, and in hospital pharmacy, in the Regional Health Administration of the Center under the guidance of Dr. Catarina Coelho.The monograph was carried out based on literature review relevant to the topic and on the analysis of spontaneous reports of adverse reactions to the quinolone class antibiotics reported to the Pharmacovigilance Unit of Coimbra, AIBILI (Association for Biomedical Re-search in Light and Image), over a five-year time horizon. Quinolones are a class of broad-spectrum synthetic antibiotics that act by inhibiting DNA synthesis, widely used in the treat-ment of bacterial infections, which prompted the increase of resistance to this group. For the analysis of the iatrogenicity caused by these drugs, 29 spontaneous reports containing 51 adverse events were considered. The cases were characterized based on primary validation criteria (description of the ADR (s), identification of the suspected drug (s), patient and noti-fier identification) and secondary (severity, causation and previous knowledge). The most frequently suspected drug was ciprofloxacin (n = 19; 65.52%). Of the total notifications, 6.90% contained simultaneously serious adverse events, unknown and with least probable relation with the suspected drug, which reflects the importance of NE in generating new knowledge about the safety profile of the drugs. The development of new quinolones com-bined with the knowledge generated in this area allows an extension of therapeutic options and the safety profile of marketed drugs.The internship reports have the structure of a SWOT (Strengths, Weaknesses, Weak-nesses, Opportunities, Threats, Threats) analysis, regarding my opinion about the adequacy of the curricular plan vis-à-vis future professional perspectives, the knowledge acquired in this first contact with professional practice, the activities carried out within these institutions and their modi operandi. From this observation it was concluded that, despite the excellent service provided by these entities, there are aspects that can be improved.
