Abstract: O contacto com uma realidade profissional, através da realização do estágio curricular em farmácia comunitária, possibilitou a integração dos conhecimentos adquiridos ao longo do MICF em contexto real e a aquisição de competências inerentes à atividade farmacêutica. Recorrendo à análise SWOT, descrevem-se os pontos fortes, os pontos fracos, as oportunidades e as ameaças que marcaram a experiência vivida na Farmácia Coimbra. Numa farmácia, a medição da pressão arterial e a cedência de medicamentos anti-hipertensores são uma prática diária comum. A hipertensão arterial é um fator de risco cardiovascular grave e afeta 42,2% da população adulta portuguesa, constituindo um sério problema de saúde pública. Por conseguinte, é fundamental instituir um tratamento farmacológico adequado e associado a alterações do estilo de vida. O doente hipertenso deve privilegiar uma alimentação saudável, embora os alimentos possam interagir com os medicamentos. Uma interação medicamentosa consiste na alteração da atividade farmacológica de um fármaco provocada pela administração anterior ou concomitante de um outro fármaco, fatores dietéticos ou hábitos sociais, tais como beber e fumar. Vários estudos sugerem que os alimentos e outros componentes da dieta podem afetar a biodisponibilidade dos medicamentos usados no tratamento da hipertensão. A interação mais bem estudada é a que se verifica entre o uso concomitante de sumo de toranja e felodipina. Demonstrou-se que o sumo de toranja produz um efeito inibitório da CYP3A4 intestinal e de transportadores específicos originando um aumento da biodisponibilidade oral do fármaco com consequente redução acentuada da pressão arterial e outros efeitos indesejados. Inúmeros estudos têm sido realizados para identificar outras interações desta natureza. Enquanto profissional de saúde, o farmacêutico deve abordar os hábitos alimentares com o doente hipertenso e recomendar a necessidade de evitar o consumo de determinados alimentos e de suprimir a ingestão do sumo de toranja e outros sumos da dieta, por exemplo, os sumos de laranja e de maçã, devido às prováveis implicações na eficácia e segurança dos medicamentos usados no tratamento da hipertensão.
The contact with a professional reality, through the conclusion of a curricular internship in a community pharmacy, allowed the integration of knowledge acquired along the MICF in real context and the acquisition of skills inherent to the pharmaceutical activity. Using the SWOT analysis, we describe the strengths, weaknesses, opportunities and threats that have marked the lived experience in Farmácia Coimbra. In a pharmacy, the measurement of arterial pressure and the provision of anti-hypertensive drugs are a common daily practice. Arterial hypertension is a major cardiovascular risk factor and affects 42.2% of the Portuguese adult population, which constitutes a serious public health problem. Therefore, it is essential to establish an adequate pharmacological therapy associated with changes in lifestyle. The hypertensive patient should focus on a healthy diet, although food may interact with drugs. A drug interaction is defined as the alteration of the pharmacological activity of a drug produced by previous or concomitant administration of another drug, dietary factors or social habits, such as drinking and smoking. Several studies suggest that food and other dietary components can affect the bioavailability of drugs used in hypertension therapy. The most studied interaction is that between the concomitant use of grapefruit juice and felodipine. It has been demonstrated that grapefruit juice produces an inhibitory effect of intestinal CYP3A4 and specific carriers resulting in an increase of the oral bioavailability of the drug with consequent significant reduction of blood pressure and other unwanted effects. Numerous studies have been conducted to identify other interactions of this nature. As a health professional, the pharmacist must address the dietary habits with the hypertensive patient and recommend the need to avoid the consumption of certain foods and to suppress grapefruit juice ingestion and other juices in the diet, e.g. orange and apple juices, due to their possible implications on the effectiveness and safety of drugs used in hypertension therapy.
