Abstract: Introdução: O défice intermédio de Alfa1-antitripsina (AAT) caracteriza-se por níveis séricos de AAT>50mg/dL em doentes com genótipos SZ, MZ, SS e MS (ou outras mutações mais raras). Apesar dos vários estudos publicados, o risco de doença pulmonar nestes doentes não se encontra bem esclarecido. Objetivos: Com este estudo retrospetivo, pretende-se avaliar a existência de alterações pulmonares e as suas características em doentes com défice intermédio de AAT, bem como a sua relação com o nível sérico de AAT e a exposição a fatores de risco. Materiais e Métodos: Realizou-se uma análise retrospetiva dos doentes com défice intermédio de AAT, com níveis séricos de AAT>50mg/dL e genótipo compatível, seguidos na consulta de Défice de AAT, no Serviço de Pneumologia B do CHUC-HG. Efetuou-se a caracterização clínica da população total e de cada um dos grupos genéticos (SS, MZ, SZ), avaliando-se a existência de diferenças entre eles em termos funcionais e de extensão do enfisema. Avaliaram-se ainda as diferenças em termos funcionais respiratórios e do Índice de Enfisema (IE) Total relativamente à exposição a fatores de risco, à sintomatologia e ao motivo de diagnóstico. Os dados foram analisados usando o programa SPSS®, versão 24.00. Resultados: A população estudada é predominantemente sintomática e diagnosticada por rastreio familiar. Na sua maioria, não possui hábitos tabágicos nem está sujeita a exposição profissional. Apenas uma pequena percentagem de doentes apresenta obstrução no Estudo Funcional Respiratório (EFR) ou IE Total>5%. Os indivíduos caso índice apresentam maiores alterações da função respiratória e IE Total superior, comparativamente aos indivíduos diagnosticados por rastreio familiar. Quanto à sintomatologia, identificou-se pior função pulmonar em indivíduos sintomáticos, por comparação aos assintomáticos. Verificou-se existir diferença estatisticamente significativa para o nível sérico de AAT entre os diferentes grupos genéticos mas não para os parâmetros funcionais respiratórios ou IE. Observou-se correlação desprezível entre o nível sérico de AAT e os parâmetros funcionais e IE Total. Relativamente à exposição a fatores de risco, verificou-se que os indivíduos fumadores e ex-fumadores, bem como os indivíduos com exposição profissional, apresentam valores superiores de IE Total e mais alterações nos parâmetros funcionais respiratórios. Na análise da exposição ambiental global, verificou-se que os doentes não fumadores e sem exposição profissional apresentam melhor função pulmonar, comparativamente com os doentes com exposição a um ou a dois fatores de risco. Conclusões: No nosso estudo, os doentes com défice intermédio de AAT são predominantemente sintomáticos e diagnosticados por rastreio familiar, sendo pequena a percentagem de indivíduos com obstrução no EFR e com IE>5%. Não há diferenças estatisticamente significativas em termos de função pulmonar ou IE Total entre os grupos genéticos definidos nem correlação das mesmas variáveis com o nível sérico de AAT. A exposição ambiental estudada - tabagismo e exposição profissional - constituiu um fator determinante para a presença de alteração da função pulmonar.
Introduction: Intermediate Alpha1-antitrypsin (AAT) deficiency is defined by serum levels of AAT>50mg/dL in patients with SZ, MZ, SS and MS genotypes (or other rare mutations). Although several studies have been published on this subject, the risk of pulmonary disease in these patients is yet to be clarified. Objectives: The aim of this retrospective study was to evaluate the existence of lung disease and its characteristics in patients with intermediate AAT deficiency, as well as its relation with serum levels of AAT and the exposure to risk factors. Materials and Methods: A retrospective analysis of patients with intermediate AAT deficiency attending the AAT deficiency appointment in the Pneumology B Department at CHUC-HG with AAT serum levels>50mg/dL and compatible genotypes, was carried out. A clinical characterization of the whole population and of each genotype group (SS, MZ, SZ) was made. Differences between them in functional parameters and in the extent of emphysema were evaluated. The differences in respiratory function and Total Emphysema Index (EI) relatively to the exposure to risk factors, symptoms and reason for diagnosis, were also analyzed. Data was analyzed using SPSS®, version 24. Results: The studied population is mainly symptomatic and diagnosed through family screening. The majority of patients are non-smokers and with no occupational exposure. Only a small percentage of patients show obstruction in lung function tests (LFT) or have an Total EI>5%. Index cases show a higher Total EI and more lung function abnormalities, compared to individuals diagnosed through family screening. In terms of symptoms, worse pulmonary function was observed in symptomatic patients, compared to asymptomatic individuals. We found differences in the AAT serum levels between genetic groups, but no differences in LFT parameters or Total EI. There was a negligible correlation between AAT serum levels and functional parameters and Total EI. Regarding the exposure to risk factors, smokers and ex-smokers, as well as individuals with occupational exposure, had higher Total EI and more lung function abnormalities. In the global environmental exposure, non-smokers and patients with no occupational exposure showed better pulmonary function relatively to individuals with exposure to one or two risk factors. Conclusion: Patients with intermediate AAT deficiency are predominantly symptomatic and diagnosed through family screening, with a very low percentage of patients with obstruction on LFT and Total EI>5%. No statistically significant differences were registered in terms of pulmonary function or Total EI between genetic groups, nor did these variables correlate with the AAT serum levels. The environmental exposure smoking habits and occupational exposure - was a determining factor for the presence of pulmonary function abnormalities.
