dc.description.abstractIntrodução: A reabsorção radicular apical externa é uma sequela iatrogénica do tratamento ortodôntico. A realização de um correto e precoce diagnóstico imagiológico é fundamental para prevenir sequelas irreversíveis que comprometem o sucesso do tratamento. A identificação de reabsorção radicular de dimensão considerável durante o tratamento ortodôntico torna possível uma alteração do plano de tratamento ortodôntico, de modo a minimizar a contínua perda de estrutura dentária radicular. Objetivo: Avaliar e comparar a precisão de diagnóstico da radiografia periapical e da tomografia computorizada de feixe cónico na deteção da reabsorção radicular apical externa associada ao tratamento ortodôntico. Materiais e Métodos: A amostra do estudo foi composta por 41 pacientes em fase de contenção ortodôntica, que frequentavam as consultas de pós-graduação de Ortodontia da Área de Medicina Dentária da Faculdade de Medicina da Universidade de Coimbra. Foram efetuados, a cada paciente, três exames radiográficos intraorais periapicais digitais e um exame de tomografia computorizada de feixe cónico, com o intuito de avaliar os incisivos maxilares e mandibulares. A análise das imagens obtidas pela tomografia de feixe cónico foi efetuada com o Software OnDemand3DApp. O diagnóstico de reabsorção radicular apical externa (presente ou ausente) foi realizado por duas Médicas Dentistas experientes e utilizado como “padrão de referência” para posterior avaliação de cada dente. Seguidamente as imagens foram analisadas por dois examinadores pós-graduados em Ortodontia. A análise da curva ROC foi utilizada para avaliar a precisão diagnóstica de cada examinador e de cada tipo de exame imagiológico. Foram calculados os seguintes valores de performance do diagnóstico: acurácia, sensibilidade, especificidade, valor preditivo positivo e valor preditivo negativo.A concordância inter e intraexaminadores foi avaliada através da estatística Kappa. Resultados: Os valores da área abaixo da curva ROC foram aproximadamente os mesmos para ambos os métodos imagiológicos na precisão de deteção da reabsorção radicular apical externa, não sendo as diferenças estatisticamente significativas (p> 0.05). Na amostra da fase 1, com todos os incisivos, 79% dos dentes apresentaram reabsorção radicular. Contudo, na fase 2, a presença de reabsorção esteve entre os 67% (incisivos inferiores e centrais) e os 75% (incisivos laterais).Conclusão: Este estudo sugere não existirem diferenças significativas na precisão entre a radiografia periapical e a tomografia computorizada de feixe cónico para a deteção da reabsorção radicular apical externa associada ao tratamento ortodôntico. A radiografia periapical continua a ser o exame primário de diagnóstico desta lesão. Contudo, caso se justifique, pode complementar-se com a tomografia de feixe cónico.por
dc.description.abstractIntroduction: External apical root resorption is an iatrogenic sequel of orthodontic treatment. The achievement of a correct and early imaging diagnosis is essential to prevent irreversible sequelae that may compromise the success of the treatment. The identification of considerable root resorption during orthodontic treatment makes it possible to alter the orthodontic treatment plan in order to minimize the continuous loss of tooth structure.Objective: To evaluate and compare the diagnostic accuracy of periapical radiography and cone beam computed tomography in the detection of external apical root resorption associated with orthodontic treatment.Materials and Methods: The study sample consisted of 41 patients in the orthodontic retention phase who attended the Orthodontic post-graduation appointments of the Dentistry Medicine Area of the Faculty of Medicine of the University of Coimbra.Three digital periapical intraoral x-ray examinations and a cone beam computed tomography scan were performed on each patient, with the purpose of evaluating maxillary and mandibular incisors. The analysis of the images obtained by CBCT was performed with OnDemand3DApp Software. The diagnosis of external apical root resorption (present or absent) was performed by two experienced Dentists and used as a "reference standard" for subsequent evaluation of each tooth. The images were then analyzed by two post-graduate examiners in Orthodontics.The ROC curve analysis was used to evaluate the diagnostic accuracy of each examiner and each type of imaging examination. The following diagnostic performance values were calculated: accuracy, sensitivity, specificity, positive predictive value and negative predictive value.Inter- and intra-examiner agreement was assessed using Kappa statistics.Results: The values of the area below the ROC curve were approximately the same for both imaging methods in the accuracy of detection of external apical root resorption, and the differences were not statistically significant (p> 0.05). In the sample from phase 1, with all the incisors, 79% of the teeth presented root resorption. However, in phase 2, the presence of resorption was between 67% (lower and central incisors) and 75% (lateral incisors).Conclusion: This study suggests that there are no significant differences in the accuracy between periapical radiography and cone beam computed tomography for the detection of external apical root resorption associated with orthodontic treatment. Periapical radiography remains the primary diagnostic examination of this lesion. However, if necessary, we can use cone beam tomography.eng
