Abstract: Introdução: A doença pulmonar obstrutiva crónica (DPOC) é uma das principais causas mundiais de morbilidade e mortalidade, constituindo um grave problema de saúde pública. A sua história natural é pontuada por exacerbações (EADPOC), que são responsáveis por um consumo considerável de recursos de saúde, nomeadamente consultas médicas não planeadas, episódios de urgência e hospitalizações. O objetivo deste estudo é caracterizar uma população de doentes com DPOC que recorre ao Serviço de Urgência (SU) e respetivos episódios de urgência, possibilitando a implementação de estratégias que resultem na redução do recurso ao SU por estes doentes. Métodos: Foi realizado um estudo retrospetivo incluindo doentes com DPOC seguidos em consulta no serviço de Pneumologia A do Centro Hospitalar Universitário de Coimbra. Destes, foram selecionados os residentes no distrito de Coimbra e que recorreram ao SU nos 6 meses prévios ou posteriores à última consulta. Os dados foram obtidos através da consulta dos processos informatizados, e incluíram variáveis demográficas e clínicas.Resultados: Foram incluídos 62 doentes com uma idade média de 74 ± 11,4 anos, sendo que 79% eram do sexo masculino. O tempo de follow-up médio foi de 10,5 meses, variando entre 6 e 12 meses. A maioria dos doentes (93,6%) apresentava uma ou mais comorbilidades. As mais prevalentes foram hipertensão arterial (74,1%), insuficiência cardíaca (35,5%) e depressão ou distúrbio de ansiedade (32,3%). Oito doentes (12,9%) foram internados por exacerbação aguda de DPOC no ano anterior ao estudo. Durante o período de seguimento, e para a totalidade dos doentes em estudo, contabilizaram-se 139 visitas ao SU. Observou-se que 42,3% dos episódios de urgência foram por doença respiratória, sendo que 51,1% das idas ao SU se deveram a comorbilidades pré-existentes. A maioria (70,5%) dos episódios terminou com alta, para o domicilio ou instituição, e 27,5% resultou no internamento do doente. Cada doente recorreu ao SU, em média, 2,7 ± 1,7 vezes por ano. A análise multifatorial identificou a presença de internamento(s) por EADPOC no ano anterior ao estudo, estadio GOLD > 2 e insuficiência cardíaca como preditores de maior número de idas ao SU por ano.Conclusão: A prevalência de comorbilidades nestes doentes é muito elevada, sendo estas responsáveis pela maioria dos episódios de urgência. Adicionalmente, verificou-se que os doentes que recorrem ao SU o fazem frequentemente. A presença de insuficiência cardíaca, estadio GOLD superior a 2 e internamento(s) por EADPOC no ano anterior ao estudo foram fatores de risco independentes para maior número de idas anuais ao SU.
Introduction: Chronic obstructive pulmonary disease (COPD) is a leading cause of morbidity and mortality worldwide. Its natural history is punctuated by acute exacerbations (AECOPD), which are responsible for a considerable consumption of health-care resources, namely unplanned medical consultations, emergency department (ED) visits and hospitalizations. The aim of this study is to characterize a population of patients with COPD who have visited the emergency department, and its ED encounters, enabling the implementation of strategies that may result in the reduction of emergency department visits by these patients.Methods: A retrospective study was carried out including patients with COPD followed in an ambulatory care regimen in the Unit of Pulmonology A of Centro Hospitalar e Universitário de Coimbra. Of these, we selected the ones who lived in the district of Coimbra and had at least one ED visit within 6 months prior to or following the last consultation. The data were obtained by consulting the patients’ electronic health record, and included demographic and clinical variables.Results: Sixty-two patients with a mean age of 74 ± 11,4 years were included, with 79% being male. The mean follow-up time was 10,5 months, ranging from 6 to 12 months. Most patients (93,6%) had one or more comorbidities. The most prevalent ones were hypertension (74,1%), heart failure (35,5%) and depression or anxiety disorder (32,3%). Eight patients (12,9%) were hospitalized for an acute exacerbation of COPD in the year prior to the study. During the follow-up period, and for all of the patients under study, 139 visits to the ED were counted. It was observed that 42.3% of the ED visits were due to respiratory disease, and 51,1% were due to pre-existing comorbidities. The majority (70,5%) of the episodes ended with the discharge of the patient, and 27,5% resulted in the patient being hospitalized. Each patient had a mean of 2,7 ± 1,7 ED visits per year. The multifactorial analysis identified hospitalization(s) for AECOPD in the year prior to the study, GOLD > 2 and heart failure as predictors of greater number of ED visits per year.Conclusion: The prevalence of comorbidities in these patients was very high and these were responsible for most of the ED visits. In addition, it was found that COPD patients who visit the ED, do so frequently. The presence of cardiac insufficiency, hospitalization(s) for AECOPD in the year prior to the study and GOLD > 2 were independent risk factors for a greater number of annual ED visits.
