Abstract: A osteomielite–uma infeção do tecido ósseo, normalmente provocada por bactérias-ocorre com alguma frequência após uma cirurgia óssea reconstrutiva ou após a inserção de uma prótese articular.Geralmente,o tratamento desta infeção compreende a administração sistémica de elevadas doses de antibióticos,por via intravenosa ou oral. Esta estratégia terapêutica,para além de ter o risco da ocorrência de efeitos adversos graves, nem sempre é eficaz,pois a fraca irrigação do tecido ósseo faz com que seja muito difícil atingir e manter,no local afetado,concentrações de antibiótico acima do nível terapêutico.Face às limitações apresentadas,este trabalho teve como objetivo a produção de membranas fibrosas de base polimérica produzidas pela técnica de electrospinning,capazes de atuarem como SLCs.Os SLCs vão possibilitar a libertação gradual e prolongada dos fármacos no local específico em que devem atuar,maximizando o seu benefício clínico e diminuindo simultaneamente os seus efeitos adversos.Uma vez que estes materiais vão estar inseridos no organismo,é necessário que estes possuam diversas caraterísticas,nas quais se destacam a biodegradabilidade e a biocompatibilidade.Assim,neste trabalho,optou-se pela utilização dos PLAs e PCL.Desta forma foram incorporados nestas membranas dois fármacos:GS-que tem como objetivo combater a possível infeção bacteriana-e DEX,que para além de minimizar uma resposta adversa ao implante.O caráter hidrofílico do GS propicia uma libertação inicial muito acentuada deste fármaco.Assim, uma possível solução para contornar este problema consiste na imobilização deste fármaco em NPSi e, por sua vez, a incorporação das nanopartículas carregadas com GS(NPSiGS) em matrizes fibrosas produzidas por electrospinning.O trabalho desenvolvido foi dividido em duas fases:a primeira compreende a imobilização do GS nas NPSi e a sua caracterização enquanto que a segunda envolveu a produção e caracterização das membranas fibrosas produzidas pela técnica de electrospinning.Nesta segunda etapa prepararam-se várias membranas,onde o GS foi incorporada nas fibras em diferentes formas: i)Na forma “livre”;ii)imobilizada nas NPSi;iii)uma parte na forma livre e outra parte imobilizada em NPSi.Em algumas membranas incorporou-se também,a par com a GS,o anti-flamatório DEX.Foram também utilizadas duas variantes da técnica de electrospinning:o electrospinnnig por blending, com a qual se obteve fibras de PLA com uma estrutura homogénea,e o electrospinnnig coaxial, onde se obteve fibras com uma estrutura núcleo-camada externa ,onde o núcleo das fibras é composto de um material diferente da camada externa.Neste último tipo de fibras a GS(nas três formas mencionadas em cima),foi incorporada no núcleo das fibras (formado por PLA),enquanto que a DEX foi incorporada na camada externa,formada por PLC.As NPSi-antes e depois do carregamento com GS-foram caraterizadas pelo método de adsorção de azoto,que permitiu a obtenção de parâmetros como a S,o Vp e o respetivo Dp.A diminuição da S bem como do Vp das NPSiGS relativamente às NPSi demonstrou que o objetivo de incorporar o GS nas NPSi foi conseguido.A quantificação de GS imobilizada nas nanopartículas foi determinada através de uma análise termogravimétrica, tendo-se obtido uma percentagem de carregamento de 43%(m/m),concluindo-se assim que o GS foi imobilizado com sucesso nas NPSi.Através da análise SEM foi possível a caracterização morfológica das membranas a partir da qual foi possível observar que todas as membranas apresentavam fibras bem formadas, aleatoriamente distribuídas sendo também possível verificar a presença de alguns beads.Nos estudos de libertação in vitro constatou-se que,para todas as membranas, o perfil de libertação do GS é mais rápido do que o perfil de libertação da DEX, tal como seria de esperar, dada a natureza hidrofóbica da DEX e a hidrofilicidade da GS.Verificou-se ainda que nas membranas que possuem os dois fármacos incorporados simultaneamente a presença do GS nas membranas altera o perfil de libertação da DEX,tornando-o mais rápido,relativamente ao perfil da DEX a partir da membrana em que esta se encontra imobilizada sozinha(PLA+DEX).Ainda nesta situação, através dos perfis de libertação do GS concluiu-se que a presença das NPSiGS na formulação contribui para uma diminuição da cinética de libertação resultando assim um perfil de libertação mais lento e gradual para o caso das membranas compostas por NPSiGS.Por fim, os resultados obtidos através das membranas produzidas por electrospinning coaxial não foram os esperados já que em vez de se obter uma libertação mais lenta e gradual de ambos os fármacos,obteve-se burst release elevados para todas as membranas.Face aos resultados alcançados pode concluir-se que as membranas fibrosas produzidas por electrospinning por blending podem ser utilizadas como SLCs no tratamento da osteomielite já que possuem a capacidade de libertarem os fármacos de uma forma controlada por um período de tempo desejado.
Osteomyelitis - a bone infection usually caused by bacteria – occurs with some frequency in the following of a bone reconstructive surgery or implantation of an articular prosthesis. Generally, the treatment of this infection includes the systemic administration of high doses of antibiotics, intravenously or orally. This therapeutic strategy, besides the risk of serious adverse effects, is not always effective, because the poor irrigation of bone tissue makes it very difficult to achieve and maintain, at the affected site, antibiotic concentrations above the therapeutic level.Considering these restrictions, this work aimed the development of polymeric fibrous implants produced by electrospinning technique, capable of localised and controlled release of antibiotics (SLCs). These SLCs will allow the controlled and sustained release of drugs in the specific site at which they are implanted, thus maximising the clinical benefit of the drugs and simultaneously reducing its adverse effects. As materials that will be inserted in the body, they must have some characteristics like biodegradability - so there will be no need for a second surgical procedure to remove the device after the treatment - and biocompatibility. In this work, two polymers were selected to fabricate the SLC: poly (lactic acid) (PLA) and polycaprolactone (PCL). Also, two different drugs were immobilized in the fibrous implants: gentamicin sulfate (GS) – whose aim is to fight a possible bacterial infection – and dexamethasone (DEX), which minimizes the risk of an eventual adverse reaction to the material and also has the capacity to promote the regeneration of the damaged bone tissue. GS’s hydrophilic character promotes a fast and extensive release at the beginning of the process. One possible solution for this problem is the immobilisation of this drug into mesoporous silica nanoparticles (NPSi). These GS loaded nanoparticles will then be introduced into the electrospinning polymer solution.This work has been developed in two phases: the first one involved the loading of the nanoparticles with GS and its characterization, while the second stage dealt with the production of the drug loaded electrospun membranes and its characterization. In this second stage several membranes have been produced, with GS being loaded in different forms into the fibers: i) entirely in its “free form”; ii) completely immobilized in NPSi; iii) part in its free form and part immobilized in NPSi. In some membranes the anti-inflammatory DEX was also added along with GS. Two different types of electro spinning techniques have also been used: the blending one, which originated PLA fibers with a homogenous structure; and the coaxial type, that allows the production of fibers with a core-shell structure, where the core material differs from the shell one. In these core-shell fibers the GS – in the three forms mentioned above – was inserted in the fiber’s core (made with PLA) while DEX was immobilized in the shell (made of PLC).Silica nanoparticles – before and after being loaded with GS – were studied using the nitrogen adsorption method, which allowed the determination of certain parameters like the specific surface area, S, pores volume, Vp, and average diameter, Dp. Reduction of S as well as of Vp on NPSiGS when compared with pure NPSi revealed GS was successfully loaded onto the NPSi. The amount of GS immobilized in the nanoparticles was determined through thermo-gravimetric analysis: results showed a 43 % (m/m) loading, thus concluding a successful GS immobilization on NPSi.Membranes’ morphological characterization was made through SEM analysis, showing that, in general, all membranes presented well-formed fibers, randomly distributed. The presence of some beads was also observed.During the 21 days period of the in vitro releasing studies, it was observed that the GS release profiles were faster than the DEX ones, for all membranes. These were expected, considering the hydrophobic nature of DEX and the hydrophilicity of GS.Results also showed that, in those membranes in which the two compounds were added simultaneously, GS changed DEX’s release profile, speeding it up when compared with the membrane where DEX stands alone (PLA + DEX). Results also showed that in these membranes NPSiGS helped to reduce GS release kinetics, thus giving a slower and balanced release profile. On the contrary, membranes made by co-axial electro spinning didn’t achieve the expected results originating, for both drugs, release profiles with accentuated burst releases. Giving the achieved results we may conclude that nano fibrous membranes produced by blended electro spinning might be used as SLCs for osteomyelitis treatment or prevention, as they help to control –in speed and time- the release of the pharmaceutical compounds.
