Title: Hemicraniectomia descompressiva no enfarte "maligno" da artéria cerebral média

Abstract: Introdução O enfarte por oclusão da artéria carótida interna ou artéria cerebral média é uma condição ameaçadora de vida e ocorre em 10 a 15% dos doentes com Acidente Vascular Cerebral isquémico. Devido à sua extensão e aparecimento de edema cerebral secundário não é rara a ocorrência de herniação sub-falciforme e transtentorial com elevação da pressão intra craneana nos 2 a 5 dias após o início dos sintomas deficitários. Subsequentemente, há uma rápida deterioração neurológica, levando a coma e morte em até 80% dos casos, apesar de terapêutica médica intensiva. A hemicraniectomia descompressiva surge como uma possível terapêutica alternativa eficaz nestes casos. É realizada com o intuito de prevenir a espiral mortífera do enfarte “maligno” da artéria cerebral média, restaurando o fluxo comprometido na área de penumbra e territórios vasculares adjacentes, normalizando a pressão intra craneana e reposicionando na linha média o tronco cerebral e diencéfalo. Objectivos e Métodos Realização de um trabalho de revisão para avaliar o potencial benefício da abordagem cirúrgica, através de hemicraniectomia descompressiva, no tratamento de doentes com enfarte “maligno” da artéria cerebral média. Os resultados foram analisados em função das variáveis idade, tempo de intervenção cirúrgico e dominância hemisférica. Procedeu-se à pesquisa no PubMed dos artigos que versam os tópicos «enfarte maligno da artéria cerebral média», «hemicraniectomia» e «cirurgia descompressiva» no período compreendido entre 1998 e 2009. Os estudos foram estratificados em função do rigor metodológico em nível A (prospectivos, multicêntricos, randomizados e controlados) e nível B (os restantes). Desenvolvimento Dos 22 artigos analisados, 4 foram classificados de nível A, correspondentes à apresentação dos resultados dos ensaios DECIMAL, DESTINY e HAMLET e à pooled analysis efectuada a partir desses três. A publicação do HAMLET fez-se acompanhar de uma meta-análise actualizada que incluiu os doentes entretanto recrutados. Os resultados destes estudos evidenciam uma redução significativa da mortalidade quando a cirurgia descompressiva é realizada nas primeiras 48 horas em doentes com menos de 60 anos (redução de risco absoluto de 50%), sem aumento do número de doentes severamente incapacitados. Nestas condições, o número necessário para tratar é de 6 para prevenir um outcome desfavorável, 2 para prevenir compromisso funcional grave ou morte e 2 para prevenir um desfecho fatal. Apesar da diminuição da mortalidade (redução de risco absoluto de 38%), não há evidência de benefício no outcome funcional quando a cirurgia é realizada até 99 horas após o início do AVC. A dominância hemisférica não é uma variável que afecte significativamente o prognóstico Conclusões Os estudos sugerem que a cirurgia descompressiva deve ser considerada em doentes até aos 60 anos que apresentem deterioração neurológica e realizada nas primeiras 48 horas após a instalação dos sintomas
Background Internal carotid or middle cerebral artery occlusion is a life-threatening condition and is found in 10 to 15% of all stroke patients. Due to its extension and the development of secondary brain edema, the occurrence of subfalcine or transtentorial herniation with elevated intracranial pressure 2 to 5 days after symptoms onset is not rare. As such, there is a rapid neurologic deterioration leading to coma and death in up to 80% of cases, despite of the best medical intensive care. Decompressive hemicraniectomy appears as an effective alternative therapy applicable in these cases. It is done with the intention to prevent the death spiral of “malignant” middle cerebral artery infarction, restoring compromised flow in the penumbra and adjacent vascular territories, normalizing intracranial pressure and the midline position of the brain stem and diecephalon. Purpose and methods Literature review about the potencial benefit of the surgical approach by decompressive hemicraniectomy, in the treatment of patients with “malignant” middle cerebral artery infarction. The results were analyzed according to age, surgical timing and hemispherical dominance in terms of mortality and functional outcome. The used method was a PubMed search of literature using the keywords “malignant middle cerebral artery infarction”, “hemicraniectomy” and “decompressive surgery” in the period between 1998 and 2009. The studies were stratified according to methodologic criteria in level A (prospective, randomized and multicentric) and B (the others). Results Of the 22 articles analyzed, 4 fulfilled the criteria for level A: the results of the clinical trials DECIMAL, DESTINY and HAMLET and the pooled analysis based on those three studies. HAMLET publication was accompanied by an updated meta-analysis that included the patients that were recruted meanwhile. The results of these studies reveal a significant reduction in the mortality rate when decompressive surgery is done within 48 hours of stroke onset, in patients younger than 60 years (absolute risk reduction of 50%), without an increase in the number of patients with severe disability. In this conditions, the number needed to treat to prevent poor outcome is 6, to prevent severe disability or death is 2 and to prevent death is 2. Despite the reduction in the mortality rate (absolute risk reduction of 38%), there is no evidence of the benefit in terms of functional outcome when surgery is performed until 99 hours after stroke onset. The hemisphere infarcted is not a variable that affects prognosis significantly. Conclusions The studies analyzed suggest that decompressive surgery should be considered in patients up to 60 years of age, who present with clinical deterioration within the first 48 hours after symptom onset
