Abstract: Introdução A doença de Behçet (DB) é uma vasculite multissistémica de etiologia desconhecida, diagnosticada, habitualmente, segundo os critérios do International Study Group for Behçet’s Disease (ISGBD) de 1990. Em 2006 foram criados os novos International Criteria for Behçet’s Disease (ICBD) e em 2010 criada uma nova proposta sobre os mesmos. Objectivo Aplicar os três sistemas de diagnóstico sobre a mesma amostra de indivíduos portadores de DB a fim de perceber qual permite uma maior precocidade diagnóstica. Métodos Foi recolhida e registada a informação clínica de doentes com o diagnóstico confirmado pelos critérios do ISGBD. A amostra foi caracterizada estatisticamente. Foram aplicados os critérios ICBD nas suas 2 versões e comparada a precocidade diagnóstica entre os três grupos de critérios. Resultados O estudo incidiu numa amostra de 50 doentes, todos eles apresentando aftose oral recorrente. A idade média da 1ª manifestação surgiu aos 14,8±10,3 anos, tendo-se registado uma demora média de 12,3±9,35 anos para estabelecer o diagnóstico de acordo com os critérios do ISGBD, de 9,2±8,9 anos com os critérios do ICBD (2006) e de 9,9±8,8 anos com a revisão destes critérios em 2010. As diferenças foram estatisticamente significativas entre os critérios do ISGBD e qualquer dos grupos de critérios do ICBD. Conclusão Qualquer das propostas de critérios de classificação do ITR-ICBD permite estabelecer um diagnóstico mais precoce de DB. Neste estudo, a proposta de 2006 é a melhor, considerando como portador de DB quem reunir um score  3 de um conjunto de manifestações que incluem a aftose genital (AG) e as lesões oculares (com um score de 2) e a aftose oral recorrente (AOR), as lesões cutâneas, o teste de patergia positivo e as manifestações vasculares (com um score de 1). Devem ser excluídas outras patologias que possam justificar aquelas lesões.
Introduction Behçet’s disease (BD) is a multisystemic vasculitis of unknown etiology, usually diagnosed according to the International Study Group for Behçet’s Disease (ISGBD) criteria, from 1990. In 2006 the new International Criteria for Behçet’s Disease (ICBD) were created and revised in 2010. Objective To apply the three sets of criteria on the same sample in order to understand which of them allows an earlier diagnosis. Methods Patients were selected with BD confirmed by the ISGBD. The sample was statistically characterized. The ICBD criteria were applied using both versions and the precocity of diagnosis was compared between the three sets of criteria. Results This study was performed over 50 patients, all presenting recurrent oral aphtosis. The mean age for the first manifestation was 14,8±10,3 years, the diagnosis could be achieved, at 12,3±9,35 years using the ISGBD criteria, at 9,2±8,9 years with the ICBD of 2006 and at 9,9±8,8 years with the ICBD of 2010. The differences were statistically significant between the ISGBD criteria and the two sets of the ICBD criteria. Conclusion Both ICBD of 2006 and its revision of 2010 proved to anticipate the diagnosis, overlapping the ISGBD criteria. In this study, the 2006 criteria was the best, diagnosing BD when 3 or more points are achieved from a set of manifestations such as genital aphtosis and eye lesions (each 2 points), recurrent oral aphtosis, skin lesions, positive pathergy test and vascular manifestations (each 1 point). Other pathologies that might justify those lesions must be excluded
