From the perspective of the social protection systems of the countries of southern Europe, we undertook an analysis of the health systems of Spain, Italy and Portugal. Through a review of health regulations, health plans and the literature on public participation experience published in recent years in Spain, Italy and Portugal, we studied the state-of-the-art of participation in the three countries. Among the results obtained, it is important to highlight the followings: a) citizen participation in decisions about health care became the focus of public policy only since the 1990; b) there have been important advances in the legal framework and recognition of the rights of patients in all three countries, and c) the degree of institutionalisation of participation is still far from satisfactory and limited to certain regions.por
