Infeção por VIH e trato gastrointestinal aspectos fisiopatológicos

Abstract

O presente trabalho expõe qual o papel do trato gastrointestinal na patogenia da infeção VIH, sendo explorados os aspetos fisiopatológicos subjacentes à transmissão da infeção, à amplificação viral e às alterações estruturais e imunológicas induzidas naquele sistema de órgãos. Foi feita uma revisão sistemática e organizada da literatura atual sobre o tema. Tendo como ponto de contacto o trato gastrointestinal, a transmissão da infeção VIH pode ocorrer por via sexual ou por transmissão vertical, tendo regra geral dois passos: transposição do epitélio e infeção de células imunitárias. A capacidade de ligação do vírus à proteína α4β7 permite a sua migração para o trato gastrointestinal. Assim, independentemente da forma de aquisição da infeção, o trato gastrointestinal é um importante local de amplificação viral, facto traduzido por uma rápida e irreversível depleção do número de linfócitos T CD4+ aí existente. Todavia, os mecanismos subjacentes às alterações estruturais e imunológicas induzidas no trato gastrointestinal pela infeção VIH ainda não se encontram totalmente esclarecidos. A lesão da barreira epitelial que ocorre no intestino possibilita a translocação de bactérias e outros produtos que perpetuarão um processo de estimulação imunitária sistémica que, por sua vez, é responsável pela evolução da infeção VIH para a cronicidade. Uma visão da evolução global da doença que tem em conta a depleção rápida e irreversível de linfócitos T CD4+ logo na fase aguda, e a exposição de diversos mecanismos passíveis de se constituírem como alvos terapêuticos representam os principais pontos de destaque do trabalho. This works exposes the role of the gastrointestinal tract in the pathogeny of HIV infection, exploring the physiopathological aspects of the transmission, viral amplification and structural and immunological alterations taking place in this system of organs. A systematic and organized review of current literature about the subject was done. Having the gastrointestinal tract as a contact point, the transmission of HIV infection can occur through sexual or vertical transmission, taking basically two steps: transposition of the epithelium and infection of immune cells. The ability of the virus to bond to α4β7 protein allows its migration to the gastrointestinal tract. In this way, regardless of the means of acquisition of the infection, the gastrointestinal tract is an important place for viral amplification, shown by the fast and irreversible depletion in the number of local CD4+ T-lymphocytes. The mechanisms of the structural and immunological alterations induced by the virus in the gastrointestinal tract during HIV infection are not completely understood yet. The epithelial barrier lesion taking place in the intestine allows translocation of bacteria and other products that will perpetuate a systemic immunological stimulation process which, by its turn, will be responsible for the evolution of HIV infection to chronicity. A model of the global evolution of the condition that takes into account the fast and irreversible depletion of CD4+ T-lymphocytes during the acute phase and the exposure of several mechanisms that could possible constitute therapeutic targets are the main topic conclusions of this works.
