Title: Procedimentos Adesivos em Odontopediatria: força de adesão e ultramorfologia das interfaces

Abstract: O aparecimento e desenvolvimento dos sistemas adesivos modificaram profundamente a vertente restauradora da Medicina Dentária, alterando os conceitos de preparo cavitário e possibilitando a realização de restaurações estéticas com maior conservação da estrutura dentária. Foram recentemente introduzidos no mercado diferentes tipos de materiais adesivos, designados por “adesivos de 6º e 7ª geração”. Diferem dos seus antecessores pela ausência de etapa isolada de aplicação do ácido, visto que o passo do condicionamento ácido foi agregado à aplicação de monómeros adesivos. Em Odontopediatria é de vital importância simplificar os passos do tratamento restaurador, constituindo os adesivos self-etch um importante aliado para este objectivo. Infelizmente, embora existam diferenças significativas entre a morfologia dos dentes decíduos e dos dentes definitivos, os sistemas adesivos têm sido estudados essencialmente para a dentição definitiva e simplesmente extrapolados para a dentição temporária. De um modo geral, independentemente do tipo de adesivo, as forças de adesão em dentes temporários são inferiores às obtidas nos dentes permanentes. Relativamente aos adesivos self-etch, enquanto as forças de adesão ao esmalte decíduo são significativamente inferiores às proporcionadas pelos adesivos do tipo etch & rinse, a adesão à dentina decídua parece ser semelhante para ambos os tipos de adesivos. Este trabalho teve como objectivo efectuar uma revisão bibliográfica sobre os adesivos self-etch aplicados em Odontopediatria e realizar um estudo de avaliação in vitro das forças de adesão à dentina de dentes decíduos, proporcionadas por diferentes materiais adesivos, bem como caracterizar por microscopia electrónica de varrimento as respectivas interfaces adesivas. Para os materiais avaliados as valores de força de adesão obtidos sugerem não existir diferenças estatisticamente significativas entre os sistemas adesivos etch & rinse e os self-etch quando aplicados em dentina de dentes decíduos. Já o compósito auto-adesivo foi o material que apresentou piores resultados. A relativa eficácia dos self-etch aliada à sua menor sensibilidade técnica, pode traduzir-se numa vantagem importante em diversas situações clínicas de Odontopediatria. The emergence and development of adhesive systems profoundly altered the aspect of restorative dentistry, changing the concepts of cavity preparation and enabling the creation of esthetic restorations with greater conservation of tooth structure. Recently, it was introduced to the market different types of adhesive materials, so called "6th and 7th generation’s adhesives". Differ from their predecessors by the lack of stage of application of acid alone, since the step of etching was added to the application of adhesive monomers. In Pediatric Dentistry is vital to simplify the footsteps of restorative treatment, constituting the self-etch adhesives an important ally for this purpose. There are significant differences between the morphology of primary teeth and permanent teeth, and unfortunately adhesive systems have been studied primarily for the permanent teeth and the results extrapolated to the temporary dentition. In general, regardless of the type of adhesive, the adhesion forces in temporary teeth are inferior to those obtained in permanent teeth. For self-etch adhesives, while the forces of adhesion to deciduous enamel are significantly lower than those offered by the ecth & rinse adhesive type, adhesion to primary dentin is similar in both types of adhesives. This study aimed to conduct a literature review on the self-etch adhesives used in Pediatric Dentistry and a study in vitro to evaluate the adhesion forces to the dentin of deciduous teeth, offered by different adhesive materials, and characterize by scanning electron microscopy the adhesive interface. The bond strength values obtained showed no statistically significant differences between the adhesive etch & rinse and self-etch when applied to the dentin of primary teeth. Already the self-adhesive composite is the material with the lower results. The relative effectiveness of self-etch combined with its lower sensitivity technique, can be a significant advantage in many clinical situations in pediatric dentistry.
