Abstract: As anfetaminas constituem substâncias psicoativas exógenas, com uma estrutura semelhante aos neurotransmissores monoamínicos endógenos e um mecanismo de ação que amplifica a ação destes. Ocorre, assim, estimulação adrenérgica, de onde resultam os seus principais efeitos: estimulante, eufórico, anorexigénio, alucinogénio e empatogénico. Foram, ao longo da história da humanidade, muito utilizadas, primariamente na sua forma natural derivada de plantas, seguindo-se, depois, a produção dos compostos sintéticos, atualmente os mais prevalentes. Inicialmente desenvolvidas com objetivo terapêutico, tornaram-se progressivamente compostos ilícitos. Mais recentemente, têm surgido as chamadas “legal highs”, que tentam contornar as imposições legais relativas à produção, tráfico e consumo das anfetaminas, e por isso mesmo, são comercializadas de forma legal em muitos países. O controlo destas substâncias, bem como dos seus precursores, que continuam a proliferar, impõe-se, ainda, como um grande desafio para as unidades de regulamentação e controlo internacional. O poli-consumo de substâncias, e de forma tipo “binge”, é o padrão de consumo mais habitual. Por este motivo, possíveis interações com outras substâncias, nomeadamente outras drogas recreativas, é de extrema importância. Hoje em dia, as suas utilizações terapêuticas são limitadas, como, por exemplo, na perturbação de hiperatividade e défice de atenção ou na narcolepsia, e o uso indevido destes derivados anfetamínicos prescritos medicamente é ainda um fenómeno mal estudado, mas do qual já existem relatos. No entanto, vários autores apontam que possíveis novos alvos terapêuticos se encontram, ainda, por descobrir. A relação risco/benefício continua a ser o pilar na decisão terapêutica. O papel do médico é essencial no reconhecimento e tratamento de situações de intoxicação, nos aspetos ligados à condução e na abordagem de situações de dependência, adição e cessação do consumo, entre outras. Daí a importância da familiarização da classe médica com estes compostos, que continuam a ter grande impacto no que respeita ao consumo mundial de substâncias.
Amphetamines are exogenous psychoactive substances, with a similar structure to the endogenous monoamine neurotransmitters and a mechanism of action that amplifies theirs. Thus, an adrenergic stimulation occurs, from which their main effects emerge: stimulant, euphoric, anorexigenic, hallucinogenic and empathogenic. They were largely used, over the history of mankind, first in their natural form derived from plants, followed by the production of synthetic compounds, nowadays the most prevalent. Initially developed with a therapeutic goal, they became progressively illicit compounds. More recently, the so called "legal highs" have emerged, which attempt to circumvent the legal impositions relatively to the production, traffic and consumption of amphetamines, and therefore are legally commercialized in several countries. The control of these substances, as well as their precursors, which continue to proliferate, remains, still, a great challenge for the control and regulation unities. The consume of multiple substances, and in a “binge” type, is the most usual pattern. For this reason, possible interactions with other substances, mainly other recreational drugs, are of extreme importance. At the present time, their therapeutic uses are limited, like, for example, in the attention deficit and hyperactive disorder or in narcolepsy, and the misuse of these amphetamine derivatives medically prescribed is still an understudied phenomenon, but of which there are already some reports. However, a variety of authors states that new possible therapeutic targets are still to be discovered. The risk/benefit relation remains the cornerstone in the therapeutic decision. The physician's role is essential in the recognition and treatment of intoxication cases, aspects related to driving and in the approach of situations of dependence, addiction and consume cessation, among others. Hence the importance of the medical class familiarization with these compounds, that remain to have a great impact in the global consumption of substances.
