Abstract: A osteoartrose é uma doença reumática caracterizada pela degenerescência da cartilagem articular (hialina), seguida pela reacção do osso subcondral através do aumento da porosidade na superfície articular, da formação de osteófitos marginais, da esclerose óssea e da eburnação. Os objectivos desta investigação incidiram-se na análise da osteoartrose nas mãos e pés procurando compreender a sua frequência, testando a relação entre o sexo, a idade à morte, a profissão, o surgimento da doença e o desenvolvimento de alterações da entese. Para o efeito, foram seleccionados 123 indivíduos da Colecção de Esqueletos Identificados da Universidade de Coimbra (séculos XIX-XX). Através dos resultados constatou-se que a osteoartrose está presente em 76,4% (n=94/123) dos indivíduos, afectando mais, nas mulheres, a mão esquerda (54,0% [n=33/61]), o pé direito (50,8% [31/61]) e o pé esquerdo (55,7% [n=34/61]) e nos homens, a mão direita (50,8% [n=31/62]). Contrariamente a outros estudos, na associação da OA com o sexo não foram encontradas diferenças estatisticamente significativas. Como se esperaria, comprovou-se através dos resultados que a idade influencia o desenvolvimento da doença, entendendo que a presença da OA aumenta com o avanço da idade. Não foi provada a associação da profissão com a doença porque não foi exequível retirar conclusões. Nas frequências dos ossos existem poucas diferenças entre as duas mãos, sendo mais atingidos, na direita, o primeiro metacárpico, as falanges proximais e mediais enquanto na esquerda foi o primeiro metacárpico e as falanges proximais. Nos pés, os ossos mais afectados pela OA foram o astrágalo e o primeiro metatársico. Neste estudo, mostrou-se existir apenas associação de alterações da entese na mão direita. Este trabalho contribuiu para ampliar o conhecimento da OA nos ossos das mãos e dos pés. Futuramente, seria importante concretizar mais trabalhos em Paleopatologia sobre a OA nas mãos e pés, valorizando a observação de todas as articulações.
Osteoarthrosis is a rheumatic disease characterized by articular cartilage degeneration (hyaline), followed by the reaction of the subchondral bone through the increasing of porosity on its surface, along with the formation of marginal osteophytes, sclerosis and eburnation. This research is focused on the analysis of osteoarthrosis in hands and feet in order to understand its frequency, looking for probable connections between sex, age at death, professions, and the emergence of the disease as well as the development of enthesis changes. To achieve our goal, 123 identified individuals were selected from the Collection of Skeletons at University of Coimbra (19th and 20th centuries). Through the samples we can see that osteoarthrosis is present in 76.4% (n = 94/123) of individuals. In women, it mainly affects the left hand (54.0% [n = 33/61]), the right foot (50.8% [31/61]) and the left foot (55.7% [n = 34/61]). As for men, the right hand is mostly affected (50.8% [n = 31/62]). Compared to other studies, the association of OA with sex differences was not statistically significant. As expected, it was shown that age has a major influence on the development of the disease, meaning that the presence of OA increases with advancing age. The association between osteoarthrosis and professional activities remains unproven, once the results were inconclusive. As it was shown, there are few differences between right and left hands, being mostly affected the first metacarpal and the proximal and medial phalanges in the right hand, whereas the first metacarpal and proximal phalanges are mostly affected in the left one. In feet, the astragalus and the first metatarsal bones were mostly affected by OA. In this study, we could only verify association of enthesis changes in the right hand. This work contributes to increasing knowledge of OA in hands and feet bones. In the future, it will be important to keep working in Palaeopathology on OA in the hands and feet, emphasizing the observation of all joints.
