Title: Os cuidados de saúde primários na prevenção do AVC

Abstract: Introdução: As doenças do aparelho circulatório, em particular as doenças cerebrovasculares são uma grande fonte de mortalidade e mobilidade. Em Portugal contribuem com grande peso para a mortalidade da população e apesar de a taxa de apresentação estar a diminuir, a sua prevalência continua alta. São na sua maioria doenças com expressão aguda, mas o seu período de incubação pode ser longo e por isso é necessário entender os fatores precipitantes ou que contribuem para o seu aparecimento. Essa compreensão permite a tomada de atitudes de forma racional e estruturada. A maioria dos AVC são primeiros eventos e grande parte pode ser prevenida. Assim a prevenção de ocorrência é primordial e é aqui que os cuidados de saúde primários são a chave pela sua visão holística do paciente, pelo acompanhamento, proximidade e cuidado que só o médico de família poder fornecer. Materiais e métodos: Foi realizada uma pesquisa bibliográfica através do uso do motor de busca do sítio da internet PubMed de artigos de 2000 a 2014, com inclusão de raras exceções justificadas pela força e relevância científica. Foram ainda consultados sítios da internet de organizações de saúde fidedignas e literatura adequada ao tema. Resultados: O papel do médico de família é incontornável no contacto da população com a saúde, sendo o elo de ligação mais forte e mais próximo. As ações de prevenção são hoje em dia um ponto essencial para a saúde das populações e tem-se verificado que a atitude expectante deve passar a ser ativa, com vista a evitar os problemas antes que estes se tornem incontroláveis ou de muito difícil gestão. Sabendo isto e pensando que o AVC é a segunda causa de morte a nível mundial, fica claro que a prevenção é essencial. Atualmente pode ser feito o cálculo do risco recorrendo a diferentes escalas de risco, mas ainda não existe modo de integrar de forma completa os diferentes problemas de um doente. Verifica-se que existem fatores de risco que aumentam o risco de ocorrência de AVC e entre eles os mais importantes são a idade, o baixo peso ao nascimento, a raça/etnicidade, o género, fatores genéticos, a atividade física, a dislipidémia, a hipertensão, a dieta e nutrição, a obesidade, a diabetes mellitus, o tabagismo, as doenças cardíacas, a estenose carotídea assintomática, o consumo de álcool, o abuso de drogas e a enxaqueca. Discussão/Conclusão: A aliança entre o conhecimento médico dos fatores de risco e a forma como o médico de família aborda os pacientes torna possível a gestão dos diferentes fatores de risco e a integração destes num paciente e em toda a complexidade da sua vida. Deve ser feita, de uma forma geral e não exclusiva do médico de família, a promoção de saúde e prevenção de doenças e em particular do AVC, como medida para o enriquecimento da saúde das populações.
Introduction: Circulatory system diseases, in particular cerebrovascular diseases are a major source of mortality and morbidity. In Portugal they account in large weight to the mortality of the population and despite the declining of the presentation rate, its prevalence remains high. They are mostly diseases with acute expression, but the incubation period can be long, so it is necessary to understand the precipitating factors or contributors to its onset. This understanding allows the embracing of rational and structured attitudes. Most strokes are first strokes and can be prevented. Thus the prevention is essential and this is where primary health care is the key for its holistic view of the patient, monitoring, proximity and care that only the family doctor can provide. Methods: A literature search through the internet using PubMed search engine was performed to find articles from 2000 to 2014, with the inclusion of rare exceptions justified by force and scientific relevance. It was also included the consultation of reliable websites of healthcare organizations and appropriate literature to the topic. Results: The family doctor's role is essential in the contact of the population with health and he is the stronger bond and closer connection in that interaction. Preventive actions are nowadays an essential point for the health of populations and has been found that expectant attitude should become active in order to avoid problems before they become unmanageable or very difficult to manage. Knowing this and thinking that stroke is the second cause of death worldwide, it is clear that prevention is essential. Currently it’s possible to do a risk assessment using different scoring systems, but there is still no way to fully integrate the different problems of a patient. There are risk factors that increase the risk of stroke among which the most important are age, low birth weight, race / ethnicity, gender, genetic factors, physical activity, dyslipidemia, hypertension, diet and nutrition, obesity, diabetes mellitus, smoking, heart disease, asymptomatic carotid stenosis, alcohol consumption, drug abuse and migraine. Discussion / Conclusion: The combination of the medical knowledge of the risk factors and the way in which the family doctor deals with patients enables the management of different risk factors and the integration of a patient and the complexity of his life. Health promotion and disease prevention in general and in particular stroke prevention must be made by the family doctor, but not exclusively by him, as a means of enriching the health of populations.
