caracterizada por uma expansão clonal das células estaminais hematopoiéticas,
contendo uma translocação recíproca entre os cromossomas 9 e 22. Esta translocação
origina o gene de fusão BCR-ABL que altera a expressão dos genes envolvidos nas vias
de sinalização celular, podendo estas sofrer alterações quando submetidas a diferentes
tratamentos.
Actualmente, as terapêuticas direccionadas para a LMC visam essencialmente
atingir a proteína de fusão BCR-ABL, como são exemplo os inibidores de tirosina
cinase que actuam no domínio cinásico.
O objectivo do presente estudo consistiu na avaliação da variação da expressão
génica que decorre na sequência da utilização terapêutica do mesilato de imatinib
(Gleevec®, Novartis) na LMC. Pretendeu-se contribuir para o esclarecimento dos
mecanismos moleculares que possam conduzir a melhoria da eficácia e eficiência
terapêutica.
Foi avaliada, por PCR em tempo real, a expressão génica relativa para 39 genes
associados a diferentes vias celulares em 9 doentes com LMC. As amostras
seleccionadas compreendiam um período de 9 meses antes e após a introdução
terapêutica do inibidor das tirosina cinases.
No conjunto de doentes analisados verificou-se uma relação inversamente
proporcional entre a expressão génica de FYN, MAP2K4, PIK3CA, RAF e FAK e
quantidade de mRNA do gene de fusão BCR-ABL. O estudo de expressão génica representa uma poderosa ferramenta para a análise
de alterações das células leucémicas, indicando o comportamento biológico específico,
o estado da doença e a resposta à terapêutica. Chronic Myelogenous Leukemia (CML) is a malignant disease characterized by a massive expansion of hematopoietic stem cells, with a reciprocal translocation between the 9 and 22 chromosomes. This translocation origin the fusion gene BCR-ABL, which changes the expression of genes involved in different cellular pathways. Different therapeutic strategies interfere with gene expression, as well. Actually, therapies for CML are more specific for the oncogene BCR-ABL, like tyrosine kinase inhibitors, which act in kinasic domain of the fusion gene. The objective of this study was to verify the difference of gene expression before and after the introduction of the Imatinib Mesylate in leukemic cells. We intended to contribute for the understanding of molecular mechanisms for a most effective and efficient therapeutic. We analyzed, by real-time PCR, the relative expression of the 39 genes, in 9 patients during 9 months, before and after of the imatinib introduction. It was verified an inverse relationship between gene expression of FYN, MAP2K4, PIK3CA, RAF and FAK and the mRNA quantity of BCR-ABL fusion gene in patients group. Gene expression represents a powerful tool to analyze genomic differences in leukemic cells, characterizing a specific biologic behavior, disease status and therapeutic response. |
